吡格列酮联合二甲双胍对2型糖尿病血脂及血浆内皮功能指标的影响

摘　　要：目的观察吡格列酮联合二甲双胍治疗2型糖尿病的临床疗效及对血脂、血浆内皮功能指标的影响。方法将明确诊断为2型糖尿病患者91例按照入院顺序随机分为观察组46例和对照组45例。对照组患者口服二甲双胍,观察组患者在对照组基础上口服盐酸吡格列酮片,两组均治疗12周。观察两组临床疗效及血脂、血浆内皮功能指标的影响。结果经过12周治疗,两组空腹血糖、2 h血糖、糖化血红蛋白均较治疗前明显降低(P<0.05),治疗后组间比较,治疗组较对照组下降更为显著(P<0.05);两组甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDLC)均较治疗前明显改善(P<0.05),两组胆固醇(TC)水平无明显变化,治疗组较对照组在TG、HDL-C、LDL-C改善更加显著(P<0.05);两组一氧化氮(NO)水平较治疗前明显提高(P<0.05),内皮素(ET)水平较治疗前明显下降(P<0.05),治疗组与对照组治疗后比较,NO、ET水平均优于对照组(P<0.05)。结论吡格列酮联合二甲双胍能显著控制血糖,降低低密度脂蛋白胆固醇和甘油三酯水平,升高高密度脂蛋白胆固醇,对血管内皮功能具有保护作用。Objective To observe the clinical effect of pioglitazone combined with metformin in the treatment of type 2 diabetes and their effects on lipid,plasma endothelial function. Methods 91 patients with type 2 diabetes were randomly divided into observation group（ n = 46）and control group（ n = 45）. The control group was treated with metformin,and the observation group was treated with oral pioglitazone hydrochloride on the basis of metformin. They were treated for 12 weeks and were observed the clinical effect,blood lipid and plasma endothelial function.Results After 12 weeks of treatment,the fasting blood glucose,2 hours postprandial blood glucose and glycosylated hemoglobin were significantly lower than before in both groups（ P〈0. 05）. And the treatment group was significantly lower than the control group（ P〈0. 05）. LDL- C,HDL- C and TG were significantly improved in the two groups compared with before（ P〈0. 05）. There was no significant change in TC levels in the two groups. The improvement of TG,HDL- C and LDL- C in the treatment group was significantly higher than that in control group（ P〈0. 05）. The levels of NO in the two groups were significantly improved compared with before（ P〈0. 05）,and the level of ET was significantly decreased（ P〈0. 05）. Both ET and NO levels in the treatment group were better than the control group after treatment（ P〈0. 05）. Conclusion Pioglitazone combined with metformin can effectively control blood glucose,reduce low- density lipoprotein cholesterol and triglyceride levels,and increase the high density lipoprotein cholesterol,and protecting vascular endothelial function.
