护理安全屏障在儿童护理不良事件管理中的应用效果

摘　　要：目的探讨护理安全屏障在儿童护理不良事件管理中的作用。方法 2013年1月至12月通过完善安全管理制度,构建安全监控网络,建立安全防范系统,持续改进护理安全质量,同时通过提倡安全为先原则,强化安全技能培养等措施,构筑组织屏障、制度屏障、预警屏障、文化屏障、人员屏障护理安全屏障,对不良事件进行预防和监控追踪,并与2012年1月至12月实施前进行对比。结果实施构筑护理安全屏障后改进流程19项,修订相关护理制度28项;新增专科护理质量评价标准24项及重点部门护理质量评价标准8项,医院护理不良事件发生率从0.30%下降到0.10%,压疮预报率提升,压疮发生率明显下降,与实施前比较(P<0.05),患儿不安全风险达到预控目标。结论构筑护理安全屏障,建立多重屏障,可提高患儿安全系数,能有效降低儿童护理不良事件发生,保障患儿接受医疗护理过程中避免不必要的伤害。Objective To explore the effects of building security perimeter in adverse events management of child nursing. Methods Measures were im- plemented including completing safety management system, building monitor seeurity network, establishing safety management system, improving quality of eare continuously, while creating nursing safety culture, emphasizing safety skills training, to build organizational barriers, institutional barriers, warning barrier and cul- tural barriers, nursing personnel security barrier for the prevention, monitoring and tracking of adverse events since January 2013, and the effectiveness was com- pared with that from January to Deeember 2012. Results After establishing the nursing safety defense, 19 proeesses and 28 related nursing regulations were modified ,24 evaluation standard of speeifie nursing quality and 8 evaluation standard of key departments＇nursing quality were added. The incidence of adverse e- vents decreased from 0.30 % to 0.10 % , the pressure sores forecast rate increased while the incidence of pressure sores decreased significantly（ P 〈 0.01 ） , unsafe risk of sick children reached pre - control objective. Conclusion Building nursing safety defenses and creating multiple barriers can improve nursing safety coef- ficient, effectively reduce the incidence of adverse events care, and protect patients from unnecessary harm while receiving medical care.
