摘　　要：目的探讨慢性心力衰竭(CHF)患者血管紧张素转化酶2(ACE2)及其抗体、血管紧张素1-7[Ang-(1-7)]与N末端脑钠肽前体(NT-proBNP)的关系。方法采用酶联免疫吸附法检测135例CHF患者和45例健康人群外周血可溶性ACE2(sACE2)、ACE2抗体及Ang-(1-7)水平,免疫荧光法检测血浆NT-proBNP浓度,分析sACE2、ACE2抗体、Ang-(1-7)与NT-proBNP的相关性。结果与健康人群相比,CHF患者sACE2、ACE2抗体、Ang-(1-7)、NT-proBNP水平随着纽约心脏病协会(NYHA)分级增高而增加(P<0.01)。CHF患者的sACE2、ACE2抗体和Ang-(1-7)水平均与NT-proBNP呈正相关(r=0.899、0.912和0.928,P<0.01)。多元线性回归分析进一步显示,sACE2、ACE2抗体、Ang-(1-7)是NT-proBNP的独立影响因素(P<0.05)。结论联合检测ACE2及其抗体和Ang-(1-7)有助于CHF的早期诊断、治疗和预后评估。Objective To investigate the association between angiotensin converting enzyme 2 （ACE2） ,ACE2 antibody ,angiotensin-（1-7）[Ang-（1-7）] and N-terminal pro-brain natriuretic peptide （NT-proBNP） in the patients with chronic heart failure（CHF） .Methods The levels of soluble ACE2 （sACE2） ,ACE2 antibody and Ang-（1-7） in peripheral blood were detected by ELISA in 135 patients with CHF and 45 healthy people . Plasma content of NT-proBNP was determined by immunofluorescence method .The correlation between sACE2 ,ACE2 antibody ,Ang-（1-7 ） and NT-proBNP was analyzed .Results Compared with healthy people ,the levels of sACE2 ,ACE2 antibody ,Ang-（1-7） and NT-proBNP were gradually increased with increasing NYHA classification in the patients with CHF （ P〈0 .01 ） .The levels of sACE2 ,ACE2 antibody and Ang-（1-7 ） were positively correlated with those of NT-proBNP in the patients with CHF（r=0 .899 ,0.912 and 0.928 , P〈0 .01） .Multiple linear regression analysis showed that sACE2 ,ACE2 antibody and Ang-（1-7） were the independent factors influencing NT-proBNP（P〈0 .05） .Conclusion Combined detection of ACE2 ,ACE2 antibody and Ang-（1-7） may be useful for the early diagnosis and treatment and the prognosis evaluation of CHF patients .
