脐带帆状附着并前置血管破裂6例临床分析

摘　　要：目的总结脐带帆状附着并前置血管破裂的临床经过,为临床提供诊治经验。方法回顾性分析6例脐带帆状附着并前置血管破裂的临床资料。结果 6例中1例经阴分娩,阴道流血量约120 m L,新生儿娩出后经抢救治疗后存活良好;4例为产程中出现胎儿宫内窘迫及时行剖宫产术终止妊娠,术前阴道流血量<50 m L,新生儿均存活良好;1例为破膜时阴道流血约150 m L,胎心急剧下降,剖宫产术前听诊胎心消失而经阴分娩一死婴。结论脐带帆状附着并前置血管破裂重在早期诊断和及时处理。一旦确诊,若胎儿存活应及早终止妊娠。Objective To concluse the clinical characteristics of ruptured vasa previa in velamentous cord insertion placenta and provide clinical experience of diagnosis and treatment. Methods Analized 6 patients of ruptured vasa previa in velamentous cord insertion placenta. Results The amount of vaginal bleeding of vaginal delivery case was about 120 m L, newborn survived well; 4 cases were terminated pregnancy by cesarean section for fetal distress, the amount of vaginal bleeding amount was less than 50 m L, all newborns survived well; 1 case occurred vaginal bleeding when membrane ruptured, the amount of vaginal bleeding amount was about 150 m L, fate heart rate fell sharply, heart disappeared before cesarean section, and a dead baby was birthed by vaginal delivery. Conclusion Ruptured vasa previa in velamentoeus cord insertion placenta was focused on early diagnosis and timely treatment.Once diagnosed, early termination of pregnancy was needed if the fetus survived.
