摘 要:目的探讨应用320排动态容积CT成像灌注成像分析副肾动脉的存在是否影响肾脏的血流灌注。方法采用320排动态容积CT对36例单侧肾脏含副肾动脉的患者行肾脏CT灌注成像,经后处理工作站计算和测量双侧肾脏皮质和髓质的血流量(blood flow,BF)、血容量(blood volume,BV)和清除率(Clearance),比较两侧肾脏间灌注参数值的差异。结果副肾动脉组肾皮质的BF值高于对照组,差异有统计学意义(P<0.05),而肾皮质BV、Clreance值及肾髓质各灌注参数两组间比较均无统计学意义(P>0.05)。在肾脏皮髓质间的比较中,实验组、对照组的肾皮质BF、BV值显著高于肾髓质(P<0.01);实验组的肾髓质Clearance高于肾皮质(P<0.05),而对照组肾脏皮髓质间Clearance值无显著差别(P>0.05)。结论副肾动脉的存在对肾脏的血流灌注影响不大。320排动态统计CT灌注成像能有效分析肾脏血流灌注情况。Objective To investigate the impacts ofmultiple renal arteries onrenal hemodynamics by using 320- slice dynamic volume CT. Methods Thirty- six patients,with unilateral kidney multiple renal arteries,underwent 320- slice dynamic volume CT perfusion imaging. The perfusion parameters as blood flow( BF),blood volume( BV) and Clearance of renal cortex and medulla in each group were calculated and measured on the workstation. And then the parameters of bilateral kidneys were compared statistically. Results BF of renal cortex in experimental group was significantly higher than that in control group( P 0. 05). No significant differences were found on BV and clearance in renal cortexbetween two groups,as well as BF,BV and clearance in renal medulla( P 0. 05). BF and BV of renal cortex were significantly higher than that of renal medulla inboth groups( P 0. 01),the result of Clearance of renal medulla was significantly higher than that of renal cortex in experimental group( P 0. 05),while there was no significantly difference between the two structure in control group( P 0. 05). Conclusions The existence of accessory renal arteries had little effects on renal perfusion.The 320- slice dynamic volume CT perfusion imaging enables toquantitative analysis ofrenal perfusion for cortex and medulla.
