摘 要:目的:探讨腰大池引流术在脑室系统出血治疗中的临床意义及可能机制。方法选取2011年03月~2014年10月我院手术治疗的符合第4届全国脑血管病学术会议制定的脑出血诊断标准的患者51例作为研究对象,按入院单双日顺序分为腰大池引流组30例(腰大池引流术治疗)和腰穿治疗组21例(常规腰穿或者脑脊液置换术治疗);治疗后对比两组患者的脑脊液转清时间,脑血管痉挛的发生例数、出院时格拉斯哥预后评分(GOS)及死亡率。结果腰大池引流组患者脑脊液转清时间、脑血管痉挛的发生例数、格拉斯哥预后评分及死亡率与腰穿治疗组对比,差异有统计学意义(P<0.05),腰大池引流组优于腰穿治疗组,差异有统计学意义(P<0.05)。结论腰大池引流术是近年来神经科新开展的最有效的治疗方法,微创、闭式、稳压、充分、持续微量引流脑脊液,其机理可能是快速廓清血性脑脊液和降低颅内压。ObjectiveIn order to investigate the sgnifi cance of Cerebrospinal fl uid drainage in the treatment of Ventricular hemorrhage.MethodsSelected in March 2011 to treated in our hospital in October 2014 conforms to the 4th National cerebrovascular disease Academic Conference on the development of cerebral hemorrhage diagnosis standard of 51 cases of patients as the research object,according to the order of admission single Sojitz points for lumbar cistern drainage group(n=30) (lumbar cistern drainage in the treatment of) and waist wear treatment group, 21 cases (routine lumbar puncture and cerebrospinal fluid replacement therapy); after treatment,compared two groups of cerebrospinal fl uid turned clear time, cerebral vasospasm occurred cases, discharge the Glasgow Outcome Score (GOS) and mortality.Results Of lumbar cistern drainage of cerebrospinal fl uid (CSF) in patients with clear time,cerebral vasospasm occurred cases,Glasgow Outcome Score and mortality associated with lumbar puncture in the treatment group compared,the difference was statistically significant(P〈0.05),lumbar cistern drainage group was better than that of lumbar puncture in the treatment group, the difference is statistically signifi cant(P〈0.05). ConclusionCerebrospinal fluid drainage is a new development most effective treatment method of Neurology in recent years, which can drainage Cerebro-Spinal Fluid by the methods of minimally invasive, closed, Stable, fully, continuous trace; The Mechanism maybe rapid clearance of bloody cerebrospinal fl uid and reduce the intracranial pressure.
