徒手胸椎椎弓根螺钉技术在特发性脊柱侧凸中的临床应用

摘　　要：目的探讨徒手胸椎椎弓根螺钉技术治疗特发性脊柱侧凸的方法、安全性及疗效。方法对18例特发性脊柱侧凸患者采用全脊柱椎弓根螺钉技术行脊柱侧凸矫形术,通过徒手置钉技术、器械矫形原理对侧凸进行矫形。结果共置入衍射172枚胸椎椎弓根螺钉,经术后CT检查,除13枚胸椎螺钉通过横突肋骨复合体进入椎体,8枚螺钉穿破椎弓根内侧壁进入椎体外,其余151枚胸椎椎弓根螺钉完全通过椎弓根进入椎体,术前平均冠状位胸主弯Cobb角为(72.80±7.98)°,术后平均Cobb角为(20.85±7.25)°,畸形矫正率为71.4%,均未发生神经、血管并发症。结论徒手胸椎椎弓根螺钉技术治疗特发性脊柱侧凸是一种安全、可靠的置钉方法,值得推广。Objective To investigate the safety, method and clinical efficacy of treatment in idiopathic scoliosis with thoracic pedicle screw technique. Methods This is a retrospective review of 18 patients with idiopathic scoliosis who underwent posterior scoliosis correction surgery using instrumentation derotation corrective techniques with complete pedicle screw instrumentation. By free-handed pedicle screw placement technique. Results A total of 172 thoracic pedicle screws were inserted, the positions of pedicle screws were evaluated by CT, all pedicle screw were inserted correctly except eight thoracic screws with perforating through the inside wall of the pedicles. The average preoperative major thoracic curve was（72.80±7.98） °and the angle was（20.85 ±7.25） °after surgery, the postoperative correction rate was71.4%. No blood vessel and spinal nerves injury were found after operation. Conclusion The treatment in idiopathic scoliosis with free-hand thoracic pedicle screw technique is a safe, reliable method, should be promoted.
