双侧接骨板治疗复杂肱骨髁间骨折43例体会

摘　　要：目的探讨切开复位双钢板内固定治疗复杂肱骨髁间骨折治疗体会。方法对2012年2月~2014年2月43例复杂肱骨髁间骨折患者住院手术治疗并取得完整随访的临床资料进行回顾分析。其中男28例,女15例;年龄22~44岁,平均38岁。骨折根据AO分型,C2型31例,C3型12例。患者采用切开复位双钢板内固定治疗,术后随访结果依据Mayo肘关节功能评分标准来评价。结果本组病例均获得随访,随访时间12~30个月,平均16.3个月。骨折均愈合,平均愈合时间4个月。依据Mayo肘关节功能评分标准,优35例,良5例,可3例,差0例。结论切开复位双钢板内固定治疗复杂肱骨髁间骨折固定牢靠,可早期锻炼,疗效确切。Objective To explore the surgical results of complex humeral intercondylar fractures treat with medial and lateral plates. Methods From February 2012 to February 2014,43 cases of patients of intercondylar fracture of humerus were treated at our institutions. The cases were reviewed retrospectively. There were 28 males, and 15 females. The average age of the patients was 38 years（range, 22 to 44 years）., There were 31 cases with type C2, 31 cases were type C3, as defined by AO classification system. The fractures were managed with open reduction, and secure internal fixation was made with medial and lateral plates. All of them were followed up, the results were evaluated with Mayo clinical scoring system of elbow joint. Results At a mean follow-up of 16.3 months（12 to 30 months）, the date shows the fractures healed in 4 months. By the Mayo rating system, 35 cases were excellent, good in 5, fair in 3 and none poor ones.Conclusion The treatment of complex humeral intercondylar fractures with medial and lateral plates is recommendable,because it is firm fixation, earlier functional exercise, more rapid recovery and fewer complications.
