Ⅱ型交锁髓内钉在胫骨远端骨折中的应用

摘　　要：目的探讨采用Ⅱ型交锁髓内钉在胫骨远端骨折中的应用疗效及优势。方法 2008年1月~2013年8月,采用Ⅱ型交锁髓内钉治疗胫骨远端骨折128例,其中男96例,女32例,年龄18~65岁,平均44.5岁,术后随访行X光片检查观察骨折愈合情况,采用AOFAS踝-后足评分系统评价踝关节功能恢复情况。结果所有患者均获得骨性愈合,愈合时间为4~9个月,平均6.5个月,未发现内固定物松动、断裂失效、切口感染等不良并发症,末次随访AOFAS评分,优105例,良21例,一般2例,差0例,优良率98%。结论采用Ⅱ型交锁髓内钉固定胫骨远端骨折,创伤小,固定坚强,利于早期功能康复,骨折愈合率高,且并发症少,是理想的内固定方式。Objective To explore the clinical outcomes of disal tibial fracture treated with intramedullary nailing（TypeⅡ）. Methods 128 patients with distal tibial fracture from Jan 2008 to Aug 2013 were retrospectively analyzedincluding96 males, 32 females, aging 18 to 65 years, average 44.5 years old,All patients were closed reduction and fixed by intramedullary nailing（Type Ⅱ）, then fracture union was evaluated according to X-rays, the function of ankle also be evaluated for each case using AOFAS scores. Results All fractures were healthed among 4 to 9months, average 6.5months,no complications of internal fixation loosening, fracture failure and infection. At the last time of follow-up, the clinical results wire assessed according to the AOFAS scores： excellent in 105, good in 21, general in 2, poor in 0, the excellent and good rate was 98%. Conclusion Intramedullary nailing（Type Ⅱ） fixation was an effective treatment for distal tibial fracture。It has fewer complications and better fuction.
