摘 要:目的 了解不同孕期孕妇血清25-羟维生素D[25-(OH)D]水平分布.方法 选择2014年12月至2015年5月在广东省妇幼保健院进行产检的正常妊娠孕妇1254例,按不同孕龄分成孕早期(〈 14周)、孕中期(14~28周)和孕晚期(〉28周)三组,其中,孕早期169人,孕中期960人,孕晚期125人.采用酶联免疫吸附法测定孕妇血清25-(OH)D水平,并比较三组结果的差异.结果 (1)孕早期孕妇血清25-(OH)D中位数为32.00(16.57~53.57) nmol/L,孕中期为34.00(17.11 ~ 64.52) nmol/L,孕晚期为33.30(17.10~70.63) nmol/L.经独立样本kruskal-wallis检验,三组孕妇血清25-(OH)D水平有统计学差异(Z=6.060,P〈0.05).(2)孕早期血清25-(OH)D缺乏和不足的发生率分别为17.75%(30/169)和81.66%(138/169),孕中期分别为12.29%(118/960)和85.94%(825/960),孕晚期分别为17.60%(22/125)和80.80%(101/125).对三组孕妇血清25-(OH)D水平分布进行秩和检验两两比较,发现孕早期与孕中期孕妇血清25-(OH)D水平分布有统计学差异,表明孕早期孕妇血清25-(OH)D营养水平低于孕中期(Z=2.162,P〈 0.05).结论 不同孕期孕妇血清25-(OH)D普遍存在缺乏和不足,孕早期更显著.Objective To investigate the serum 25-hydroxy vitamin D [25-(OH)D] levels in pregnant women in different periods.Methods 1254 pregnant women were enrolled into this study during their visit at Guangdong Women and Children Hospital between December, 2014 and May, 2015.The subjects were divided into three groups according to pregnant periods: first trimester group (〈14 weeks), second trimester group (14-28 weeks), and third trimester group (〉28 weeks).Serum 25-(OH)D levels were measured by ELISA.The differences of 25-(OH)D levels were compared between these three groups.Results (1)The median of serum 25-(OH)D was 32.00 (16.57-53.57) nmol/L in first trimester group, 34.00 (17.11-64.52) nmol/L in second trimester group, and 33.30 (17.10-70.63) nmol/L in third trimester group,with statistical differences between these three groups showed by kruskal-wallis test (Z=6.060, P〈0.05).(2)The rates of 25-(OH)D deficiency and insufficiency were 17.75% (30/169) and 81.66% (138/169) in first trimester group, 12.29% (118/960) and 85.94% (825/960) in second trimester group, and 17.60% (22/125) and 80.80% (101/125) in third trimester group, respectively.There was a statistical difference in the distribution of serum 25-(OH)D levels between first and second trimester group (Z=2.162, P〈0.05).Conclusions Vitamin D deficiency and insufficiency are common in women in different pregnant periods,especially in first trimester.
