右美托咪定静脉麻醉对高血压恢复期血流动力学的影响

摘　　要：目的 探讨右美托咪定静脉麻醉对高血压患者恢复期血流动力学、镇痛评分及躁动情况的影响.方法 将行气管插管全麻开腹手术的高血压患者98例随机分为研究组（n=49）和对照组（n=49）.研究组右美托咪定静脉麻醉,对照组靶控输入丙泊酚.统计两组恢复期血流动力学、镇痛评分、拔管质量及躁动情况.结果 研究组拔管后10 min镇静评分明显高于对照组,差异有统计学意意义（P〈0.05）;两组治疗前HR、SBP、DBP对比差异无统计学意义（P〉0.05）;研究组T2、T3、T4时刻HR、BP、DBP明显低于同时刻对照组,差异有统计学意义（P〈0.05）;研究组拔管质量明显优于对照组,差异有统计学意义（P〈0.05）;研究组术后躁动发生率明显低于对照组,差异有统计学意义（P〈0.05）.结论 右美托咪定用于高血压全麻患者恢复期具有稳定血流动力学的作用,拔管质量好,镇痛效果好,且术后躁动少,是一种理想的高血压全麻患者手术麻醉药物.Objective To investigate the influence of anesthesia by intravenously given dexmedetomidine on the hemodynamics, analgesic score, and restlessness in hypertensive patients during recovery period.Methods 98 hypertensive patients undergoing endotracheal intubation laparotomy were randomly divided into a research group （n=49） and a control group （n=49）.The research group were intravenously given dexmedetomidine for anesthesia and the control group target-controlledly input propofol.The hemodynamics, analgesic score, extubation quality, and restlessness during recovery period in both groups were statistically analyzed.Results The analgesic score was significantly higher in the research group than in the control group 10 min after extubation, with a statistical difference （P〉0.05）.Before the treatment, there were no statistical differences in HR, SBP, and DBP between these two groups （P〈0.05）.At T2, T3, and T4, HR, BP, and DBP were significantly lower in the research group than in the control group, with statistical differences （P〈0.05）.The extubation quality was significantly higher and the incidence of restlessness was significantly lower in the research group than in the control group, with statistical differences （P〈0.05）.Conclusions Anesthesia by dexmedetomidine for hypertensive patients during recovery period can stabilize hemodynamics and decrease the incidence of restlessness after operation and has good extubation quality and good analgesic effect, so it is an ideal operative anesthesia drug for hypertensive patients.
