摘 要:目的探讨辛伐他汀保护脓毒血症大鼠心肌损伤的可能机制。方法 SD大鼠45只随机分为对照组、脓毒血症组和辛伐他汀+脓毒血症组(观察组)各15只,对照组和脓毒血症组给予生理盐水灌胃2周,观察组给予辛伐他汀(20mg/kg)灌胃2周;脓毒血症组和观察组大鼠行盲肠结扎穿孔法制作脓毒血症模型,对照组大鼠不做处理;术后48h取各组大鼠心肌,行组织病理学检查观察心肌组织的病理改变情况,采用ELISA法检测心肌组织肿瘤坏死因子-α表达水平,采用Western blot法检测心肌组织高迁移率族蛋白-1表达水平,采用TUNEL法检测心肌凋亡指数。结果脓毒血症组大鼠心肌纤维水肿、疏松,间质充血水肿,胶原纤维增多,部分内皮细胞水肿脱落,观察组心肌组织改变较脓毒血症组轻;脓毒血症组心肌组织肿瘤坏死因子-α[(47.34±0.47)ng/L]、高迁移率族蛋白-1[(0.85±0.01)μg/L]水平和心肌凋亡指数[(21.57±2.67)%]明显高于观察组[(28.16±1.40)ng/L、(0.42±0.02)μg/L、(7.82±1.04)%]和对照组[(21.54±0.83)ng/L、(0.20±0.03)μg/L、(2.58±0.82)%](P<0.05),观察组高于对照组(P<0.05)。结论辛伐他汀可减少脓毒血症大鼠心肌组织肿瘤坏死因子-α、高迁移率族蛋白-1表达水平及心肌凋亡,对脓毒血症心肌抑制有保护作用。Objective To explore the mechanism of simvastatin in preventing myocardial injury in septic rats. Methods Forty-five male SD rats were randomly divided into 3 groups: control group, sepsis group, simvastatin+ sepsis group (observation group), with 15 rats in each group. Control group and sepsis group received gavage with normal saline for two weeks, and observation group received gavage with 20 mg/kg simvastatin once a day for two weeks. Sepsis group and observation group were performed cecal ligation and puncture and control group received no treatment. The myocardial tissues were obtained and the pathological changes were observed 48 hours after operation. ELISA method was used to detect the expression of tumor necrosis factor-alpha (TNF-α), Western blot was used to detect the expression of high mobility group protein-1 (HMGB-1), and TUNEL method was used to detect myocardial apoptosis index (AI). Results HE staining showed myocardial fibers edema, osteoporosis, interstitial hyperemia/edema, increased collagen fibers, and partial endocardial endothelial edema in sepsis group. Observation group had less myocardial pathological changes than sepsis group. TNF-α, HMGB-1 and AI were the highest in sepsis group ((47.34±0.47) ng/L, (0.85±0.01) μg/L, (21.57±2. 67)%), followed by those in observation group ((28. 16±1.40) ng/L, (0.42±0.02) μg/L, (7. 82± 1.04) %) and control group ((21. 54±0. 83) ng/L, (0. 20±0. 03) μg/L, (2. 58±0. 82) %) in turns (P〈0.05). Conclusion Simvastatin can reduce the expressions of TNF-α, HMGB-1 and AI of myocardial tissue of septic rats, and has a protective effect on myocardial inhibition.
