延续性护理对原发性肝癌射波刀治疗后病人生活质量影响的研究

摘　　要：[目的]探讨延续性护理对原发性肝癌射波刀治疗后病人生活质量的影响。[方法]选取符合诊断标准的肝癌病人212例,随机分为观察组107例和对照组105例,对照组射波刀治疗后给予常规护理,观察组在此基础上实施延续性护理,两组于出院前1周（干预前）、出院后3个月（干预后）采用0-10数字疼痛强度分级法（NRS）和总体幸福感量表（GWB）、体力状况量表（ECOG）以及肿瘤病人生活质量量表（QOL）对病人进行评估并比较。[结果]观察组干预后NRS评分及ECOG评分较干预前明显降低,而GWB评分及QOL评分较干预前明显升高（P〈0.05）;对照组干预后ECOG评分及NRS评分也有一定改善（P〈0.05）,但较观察组差。两组干预后NRS评分、ECOG评分、GWB评分及QOL评分比较差异均有统计学意义（P〈0.05或P〈0.01）。[结论]出院后延续护理可有效减轻射波刀治疗后病人的疼痛程度、促进病人体能的恢复,病人的自我护理能力得到了提高,同时可有效提高病人的总体幸福感及生活质量。Objective：To probe into the influence of continuity of care on the quality of life of patients with primary liver cancer after CyberKnife treatment.Methods：A total of 212 patients with liver cancer meeting the diagnostic criteria were selected and randomly divided into observation group（107cases）and control group（105cases）.The patients in control group received routine nursing after CyberKnife treatment,besides that the patients in observation group received continuity of care.The patients in both groups were evaluated and compared with 0-10digital pain intensity score（NRS）,GWB,ECOG and QOL of cancer patients respectively in a week before discharging（before intervention）and 3 months after discharging（after intervention）.Results：The NRS and ECOG score of liver cancer patients in observation group after intervention were significantly lower than that before intervention,and the GWB score and QOL score were significantly higher（P〈0.05）;while the ECOG score and NRS score in control group after intervention were also improved（P〈0.05）.After intervention,there was statistically significant difference in NRS score,ECOG score,GWB score and QOL score between both groups（P〈0.05 or P〈0.01）.Conclusion：After discharging,continuity of care can effectively reduce the pain of patients after CyberKnife treatment,promote the recovery of patients＇ physical fitness,and improve the self-care ability of patients,at the same time can effectively enhance the general well-being and quality of life of patients.
