标准桃金娘油口服联合地塞米松鼓室内注射治疗分泌性中耳炎50例

摘　　要：目的观察标准桃金娘油肠溶胶囊口服联合地塞米松鼓室内注射治疗分泌性中耳炎的临床疗效。方法将医院收治的分泌性中耳炎100例患者随机分为对照组和观察组,各50例。两组患者均给予常规治疗,对照组患者在此基础上给予鼓膜穿刺抽液,地塞米松注射液鼓室内注射5 mg;盐酸麻黄碱滴鼻液滴鼻,2滴/次,3次/日;头孢拉定胶囊口服,每次0.5 g,3次/日。观察组患者在对照组基础上加服标准桃金娘油肠溶胶囊,每次300 mg,3次/日。两组患者均连续治疗4周。结果治疗后,观察组临床总有效率为87.50%,明显高于对照组的70.00%(P<0.05);观察组患者不同频率下纯音听阈测定气导听力、鼓室压及声阻抗鼓室压曲线改善程度明显优于对照组(P<0.05);治疗过程中,两组患者不良反应均较轻微,发生率无显著性差异(P>0.05),血、尿常规及肝、肾功能均未出现明显异常。结论标准桃金娘油肠溶胶囊联合地塞米松鼓室内注射可有效缓解耳闷塞、耳鸣等临床症状,并能显著提高患者纯音听阈测定气导听力水平,提高鼓室压,有效改善声阻抗鼓室压曲线,促进患者听力恢复。Objective To observe the clinical effect of myrtol standardized enteric coated soft capsules combined with intratympanic dexam- ethasone injection for treating secretory otitis media. Methods Totally 100 patients with secretory otitis media admitted to the hospital were randomly divided into the control group and the observation group, 50 cases in each group. The two groups were given routine treatment,on this basis, the control group were given tympanic membrane puncture,dexamethasone （intratympanic injection of 5 mg）;hy- drochloric acid ephedrine nasal drops （nasal drip 2 drops/time,3 times daily）;cephalosporins （oral,0.5 g/time,3 times daily）;the ob- servation group was added myrtol standardized enteric coated soft capsules （oral,300 mg/time,3 times daily）on the basis of the control group. The two groups were treated continuously for 4 weeks. Results After treatment, the total effective rate of the observation group was 87.50% ,which was significantly higher than 70. 00% of the control group （ P 〈 0.05） ;the improvement of air conduction hearing determined by pure tone hearing threshold under different frequency, middle ear pressure and acoustic impedance middle ear pressure curve in the observation group were significantly better than those of the control group （P 〈 0. 05 ） ; during the treatment, the adverse reactions in the two groups were mild,the incidence had no statistical difference （P 〉 0. 05）;there were no obvious abnormal appeared in blood routine, routine urine examination, liver function, kidney function. Conclusion Myrtol standardized enteric coated soft capsules combined with intratympanic dexamethasone injection for treating secretory otitis media can effectively alleviate the ear stuffy plug and tinnitus,and can significantly improve air conduction hearing level determined by pure tone hearing threshold, improve tympanic pressure and the acoustic impedance of the tympanic cavity pressure curve,promote the recovery of patients＇ hearing.
