Percutaneous Cinobufotalin Injection under Ultrasonography Combined with Transcatheter Arterial Chemoembolization on Treating Portal Vein Tumor Thrombus

摘　　要：目的探讨超声介入华蟾素联合肝动脉化疗栓塞（TACE）对门静脉癌栓的临床疗效。方法 47例肝癌伴门静脉癌栓患者随机分为2组,治疗组25例,采用超声介入华蟾素联合TACE治疗;对照组22例,单纯接受TACE治疗。观察患者治疗前后癌栓大小、生活质量、外周血实验室指标的变化,治疗后随访6-24个月,统计生存率。结果治疗组治疗后总有效率为76.0%,优于对照组45.5%;治疗组1、2年生存率分别为60.0%、36.0%,高于对照组的27.3%、9.1%;治疗组在改善生活质量方面明显优于对照组;治疗后治疗组外周血白细胞（WBC）、CD3^＋、CD4^＋、CD4/CD8、NK均高于对照组,而血清总胆红素（TBIL）、丙氨酸氨基转移酶（ALT）均低于对照组。结论超声介入华蟾素联合TACE治疗门静脉癌栓可以起到抑制癌栓生长、延长生存期、提高生活质量、改善肝功能和免疫状态的作用。Objective To evaluate the clinical effect of percutaneous cinobufotalin injection under ultrasonography combined with transcatheter arterial chemoembolization（ TACE） treatment of portal vein tumor thrombus. Methods The 47 patients with primary hepatocellular carcinoma with portal vein thrombosis were randomly divided into two groups. In the treatment,25 patients received percutaneous cinobufotalin injection under ultrasonography after TACE;and 22 patients in the control group were treated with TACE simply. The size of tumor thrombus,quality of life and peripheral blood index of laboratory were observed before and after treatment. The follow-up lasted 6 to 24 months,and the survival rate was calculated. Results The effective rate in the treatment group was 76%,which was superior to the control group; The survival rate of 1 year and 2 year in the treatment group was 60% and 36% respectively,which was higher than that of the control group（ 27. 3% and 9. 1%）; The life quality of patients in the treatment were improved more significantly than those in the control group; The WBC,CD3^＋,CD4^＋,CD4/ CD8,NK indicator of the treatment group were higher than those of the control group after treatment,the liver function such as TBIL and AIT were lower than those of the control group. Conclusion The treatment for portal vein tumor thrombus with percutaneous cinobufotalin injection under ultrasonography combined with TACE can not only inhibit the proliferation of tumor thrombus and prolong the survival time,but also protect liver function and improve quality of life.
