焊工血清铁、血糖和血脂水平及其影响因素

摘　　要：目的探讨电焊工人血清铁负荷及血糖、血脂状况。方法对电焊工和对照组人员各100人进行一般情况调查,以及血清铁、转铁蛋白、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)等指标的测定。结果焊工组血清铁水平明显低于对照组(P<0.05);两组的FPG浓度差异无统计学意义;在血脂方面,焊工组TG、LDL-C均值都明显低于对照组(P<0.01),HDL-C则高于对照组(P<0.01)。多元线性逐步回归分析结果显示,焊工组中最终进入模型对FPG产生影响的主要是年龄;对TG产生影响的是主要是体重指数(BMI);对LDL-C产生影响的主要是血清铁和工龄,随着血清铁水平的升高和工龄的增长,LDL-C会升高。焊工组中未发现TC和HDL-C与研究因素存在明显相关关系。结论焊工组的血清铁含量水平并未因接触电焊烟尘中的氧化铁而升高,反而明显低于对照组。焊工组的血清铁可能对低密度脂蛋白产生影响。影响到血脂、血糖的因素中,年龄和BMI的影响较为明显。工作人群应采取良好的生活方式控制体重,使身体更加健康。[Objective]To investigate the levels of serum iron,blood glucose and blood lipids of welders. [Methods]A total 100 welders and 100 controls were enrolled in this study. Serum levels of iron,transferrin,fasting blood-glucose（FBG）,triglyceride（TG）,total cholesterol（TC）,high-density lipoprotein（HDL-C）,low-density lipoprotein（LDL-C）were determined,as well as general conditions.[Results]The level of blood iron of welders was lower than that of control group significantly（P〈0.05）. There was no significant difference on FBG between the 2 groups. In terms of blood lipids,welders＇ average TG and LDL-C were significantly lower than the control group（P〈0.01）,but their level of HDL-C was higher than that of control group（P〈0.01）.Multiple stepwise regression analysis results showed that age might be an influencing factor for FPG,and BMI might be an influencing factor for TG. In additional,LDL-C might be impacted by serum iron and by service length. LDL-C might rise with the increase of serum iron levels and the extension of the service length. In the welder group,no significant correlation was found between HDL-C and TC with these factors. [Conclusion]In this study,exposure to ferric oxide of welding dust does not lead to any elevation in the levels of serum iron in welder group;on the contrary,it has been found significantly lower than the control group in the study. Welders＇ serum iron may have a negative impact on LDL-C. The important factors which affect blood lipid and blood glucose are age and BMI. To keep healthy,working people should have a good life style and control weight.
