子宫内膜异位症盆腔粘连的超声诊断

摘　　要：目的经阴道超声评估子宫内膜异位症(内异症)患者盆腔粘连情况。方法选择2013年2月—2014年8月临床上疑为盆腔内异症在我院行腹腔镜手术的134例患者。所有病例术前行经阴道超声检查。结果经阴道超声诊断卵巢粘连的敏感性为77.4%,重度卵巢粘连的敏感性为81.2%;诊断子宫直肠陷凹粘连的敏感性为75.0%,子宫直肠陷凹完全封闭的敏感性为82.4%。经阴道超声与腹腔镜比较诊断卵巢粘连与子宫直肠陷凹Kappa值分别为0.70、0.74。结论经阴道超声诊断卵巢粘连及子宫直肠陷凹粘连与腹腔镜比较有较好的一致性,术前经阴道超声盆腔粘连的评估对手术范围的评估及进行有重要的指导意义。Objective To study the value of transvaginal ultrasound scanning（TVS） in the diagnosis of pelvic adhesions in endometriosis. Methods 134 women who were suspected pelvic endometriosis underwent preoperative TVS and laparosco- py from February 2013 to August 2014. Results The sensibility of pre - operative TVS for onarian adhesions is 77.4%, while the sensibility of TVS for severe ovarian adhesions is 81.2%. The sensibility of TVS for pouch of Douglas（POD） ad- hesions is 75.0%, while the sensibility of TVS for complete obliteration of POD is 81.2%. Interobserver agreement of ova- ry adhesions between TVS and laparoscope was good（ Kappa = 0.70）. It is the same as recto- uterine fossa（ Kappa = 0. 74）. Conclusions Interobserver agreement of ovary adhesions and POD between TVS and laparoscope was good. The eval- uation of pre - operative TVS for pelvic adhesions provide a better overall assessment of the severity of the disease and aid in planning of surgery.
