97例改良式B-lynch缝合剖宫产效果分析

摘　　要：目的探讨改良式B-lynch缝合术在高危产后出血剖宫产术中预防性应用的效果与价值。方法选择存在高危产后出血因素行剖宫产的产妇154例,分为3组,研究组44例术中胎盘娩出后立即预防性采用改良式B-lynch缝合术,对照组1选取53例术中出血多时再行改良式B-lynch缝合止血,对照组2选取57例采用传统方法治疗。比较3组术中术后出血量、手术时间及术后并发症发生率。结果研究组术中出血量、术后24小时内出血量分别为(472.5±131.4)m L、(215.0±62.9)m L均少于对照组2(744.7±154.4)m L、(410.5±123.0)m L,差异有统计学意义(P<0.05);研究组术中出血量少于对照组1(718.7±162.2)m L(P<0.05);但3组在手术时间和术后并发症比较上差异无统计学意义。结论改良式B-lynch缝合术具有操作简单、止血迅速和安全等优势,在高危产后出血病例的剖宫产术中尽早应用。Objective To investigate the value of prophylactic use of modified B-lynch suture in cesarean section at high risk of postpartum hemorrhage. Methods 154 puerpera who deliveried by cesarean section for the high risk factors of postpartturn hemorrhage were recruited. They were divided into 3 groups, the study group had 44 cases who were performed intraoperative prophylactic modified B-lynch suture, the 53 patients of the control group lwere used modified B-lynch suture when bleeding more, and the control group 2 of 57 patients were used the traditional method of treatment for hemostasis. Then compared intraoperative and post operative blood loss volume, operative time and postoperative complication rate of the three groups. Resu Its The intraoperative bleeding volume and the postoperative blood loss volume of the control groups were both more than that in study group, the difference were statistically significant （P〈0.05）, while the operative time, postoperative complications of the three groups had no statistically difference. Conclusion The modified B-lynch suture is simply operated, rapid hemostasis and safe. Application of modified B-lynch suture in cesarean section with high risk of postpartum hemorrhage as early as possible is feasible and effective.
