关节镜清理术结合去神经化对原发性膝关节骨关节炎患者术后症状及关节功能恢复的影响

摘　　要：目的 探讨关节镜清理术联合去神经化对原发性膝关节骨关节炎患者术后症状及关节功能恢复的影响.方法 选取我院收治的原发性膝关节骨关节炎（KOA）患者72例作为研究对象,采用数字随机对照表法将患者分为对照组和观察组,对照组予以关节镜清理术,观察组予以关节镜清理术联合去神经化,行膝关节功能评分（HSS）和视觉模拟评分（VAS）评估两组术后关节功能恢复情况及术后并发症发生率.结果观察组术后1年优良率为94.44％,较对照组的77.78％高,差异有统计学意义（x2=4.18,P＜0.05）.观察组术后6个月、术后1年VAS评分较对照组低,HSS评分较对照组高,差异有统计学意义（P＜0.05）.结论 关节镜清理术结合去神经化治疗KOA,能够减少支配病变关节滑膜的神经,有效缓解疼痛症状,且不会增加术后并发症,安全有效.Objective To explore the effect of arthroscopic debridement combined with denervation on postoperative symptoms and joint function recovery of patients with primary knee osteoarthritis.Methods 72 patients with primary knee osteoarthritis （KOA） in our hospital were selected as the research object and divided into control group and observation group according to random number table.Control group was treated with arthroscopic debridement, while observation group was treated with arthroscopic debridement combined with denervation.Assessed postoperative joint function recovery with knee joint function （HSS） score and visual analogue scale （VAS） score, and the occurrence rate of postoperative complications in two groups.Results The excellent and good rate of observation group 1 year later was 94.44%, which was higher than 77.78% of control group, with statistically significant difference （P〈0.05）.6 months and 1 year after operation, VAS score of observation group were lower than those of control group, HSS score were higher than those of control group, with statistically significant differences （P〈0.05）.Conclusions Arthroscopic debridement combined with denervation in the treatment of KOA can reduce the innervation of synovium of diseased joint, effectively alleviate the symptom of pain, and does not increase postoperative complications, which is safe and effective.
