烧伤患者血清学指标的动态变化及其对并发MODS的诊断价值

摘　　要：目的观察烧伤患者血清中肌红蛋白（Mb）、肌酸激酶（CK）、C反应蛋白（CRP）、中性粒细胞百分比（NET％）和白细胞介素-6（IL-6）与患者并发多器官功能障碍综合征（MODS）的关系。方法将2013年6月～2014年8月住院的烧伤患者（烧伤面积〉15％）90例作为观察组，并根据烧伤面积的不同进一步分为3组，分别对3组患者伤后第1、3、7天血清Mb、CK、CRP、NET％和IL-6进行检测，同法测定100例健康体检者血清相应指标作为对照组。烧伤患者中并发MODS者28例。结果烧伤患者伤后血清Mb、CK和IL-6的浓度均随着治疗后时间的延长而下降；烧伤并发MODS组血清Mb、CK、CRP、NET％和IL-6水平显著高于非MODS组。结论血清Mb、CK、CRP、NET％和IL-6的动态变化与烧伤患者病情严重程度、变化趋势及预后具有良好相关性，Mb和CK对MODS的早期诊断、治疗具有临床指导意义。Objective To investigate the dynamic changes of serum myoglobin（Mb）, creatine kinase（CK）,C-reactive protein （CRP）,NET%,IL-6 and their clinical significance in burn patients. Methods The serum Mb, CK, CRP, NET% and IL-6 were measured in 90 burn patients and compared with 100 cases of normal reference group. The index of the vein blood collected from the burn patients, including 28 cases complicated with multiple organ dysfunction syndrome （MODS）, were analysed on days 1, 3, and 7 after burn accidents. Results The levels of Mb, CK and IL-6 decreased af- ter antibiotic therapy, and the levels of Mb, CK, and IL-6 were notably related tO burned area and time. Mb, CK and IL- 6 levels of MODS patients were obviously higher than of non MODS group. Conclusion The serum levels of Mb,CK, CRP,and IL-6 might serve as a valuable index for supervising burn patients status. Both Mb and CK are of significance in diagnosis and therapy of MODS.
