解毒洗药溻渍治疗对下肢静脉性溃疡疮周微循环的影响

摘　　要：目的：探讨解毒洗药溻渍治疗对下肢静脉性溃疡疮周微循环的影响。方法：选取下肢静脉性溃疡住院患者54例,随机分为治疗组38例,对照组16例;两组创面均常规消毒后湿敷庆大霉素生理盐水纱布,治疗组在换药前应用解毒洗药溻渍治疗患肢（30±2）min,疗程为4周;对比两组疗效,检测治疗前后疮周血流灌注量和经皮氧分压。结果：治疗组治愈率28.95%,显愈率71.05%,有效率97.37%;对照组治愈率18.75%,显愈率43.75%,有效率75.00%,治疗组效果优于对照组。治疗4周后,治疗组血流灌注量基线值、加热后增加率及经皮氧分压均高于治疗前（P〈0.05）;治疗组血流灌注量基线值、加热后增加率及经皮氧分压均高于对照组（P〈0.01～0.05）。结论：解毒洗药溻渍治疗下肢慢性溃疡可通过提高创面局部血流灌注量和氧分压提高疗效。ObjectiveTo investigate the clinical efficacy of detoxifying lotion on venous ulcer of lower ex-tremity.MethodsFifty-four hospitalized patients with venous leg ulcers were randomly divided into treat-ment group（38 cases） and control group（16 cases）.Both groups had been treated with wet compress gentamicinphysiological saline after clean dressing,but the treatment group were fumigated the affected limbs with detoxify-ing lotion before dressing change.The course of treatment was 4 weeks.The efficacy of treatment were comparedthe amount of detected periwound perfusion and transcutaneous oxygen partial pressure changes between twogroups.ResultsIn the control group,the recovery rate was 18.75%,significant improvement rate was 43.75%and effective rate was 75%;while in the treatment group,the above rates were 28.95%,71.05% and 97.37% re-spectively.Treatment effectiveness of treatment group was much better than that of control group.After 4 weeksof treatment,in treatment group the blood perfusion of the baseline value,the increased rate after heating andtranscutaneous oxygen pressure were all higher than those before treatment.In the treatment group,the bloodperfusion of the baseline value,increased rate after heating and transcutaneous oxygen pressure were all higherthan those in the control group（P 〈0.01～0.05）.ConclusionDetoxifying lotion can obviously improve micro-circulation around the ulcer and enhance local blood perfusion and oxygen partial pressure.
