外伤性胰腺炎的CT影像表现特点分析

摘　　要：目的分析外伤性胰腺炎的CT影像表现特点。方法选取湖北省咸宁市中心医院2012年1月-2015年3月收治的70例胰腺炎患者作为研究对象,其中外伤性胰腺炎患者(观察组)20例,普通性胰腺炎患者(对照组)50例,均接受CT检查,比较2组患者CT影像表现。结果观察组患者单个CT征象者占65.00%,多个CT征象者占35.00%;对照组患者单个CT征象者占34.00%,多个CT征象者占66.00%,2组患者在CT征象数目上比较,差异有统计学意义(P<0.05)。观察组患者小网膜积液、腹腔积液、周围脂肪间隙密度增高、胰腺形态大且结构不清检出率均明显低于对照组,差异有统计学意义(P<0.05)。结论 CT检查对外伤性胰腺炎与普通性胰腺炎的鉴别诊断有一定的价值,可依据腹部外伤史、实验室检查及CT影像表现对外伤性胰腺炎进行诊断。Objective To analyze the features of the CT image manifestation of traumatic pancreatitis. Method To select 70 cases of pancreatitis patients hospitalized at Xianning Central Hospital of Hubei Province from January 2012 to March 2015 as object of study, among whom 20 patients of traumatic pancreatitis were desig- nated as observation group, and 50 patients of common pancreatitis were designated as comparison group. Both groups received CT examination and their CT image manifestations were compared. Results In the observation group, the single CT sigh made up 65.00% of the patients, and multiple CT signs accounted for 35.00% of the patients. In the comparison group, the figures were 34.00% and 66.0% respectively. The comparative differ- ences in the CT sign numbers of the two groups were statistically significant （P〈0.05）. The detection rates of fluid collection in lesser sac, seroperitoneum, heightened gap density of peripheral fat, and larger sizes and blurred form of pancreas of the observation group were all markedly lower than those of the comparison group, and the differences of the two had statistical significance （P〈0.05）. Conclusion CT examination has certain value for the detection and differentiation of traumatic pancreatitis and common pancreatitis, and diagnosis of traumatic pancreatitis can be made from records of abdominal injuries, lab examination, and CT image mani- festations.
