摘  要:目的了解医院重症监护病房(ICU)分离的MRSA菌株SCCmec基因型及耐药特征。方法通过多重PCR进行SCCmec基因分型;采用AST GP67药敏试剂盒进行药敏检测。结果 120株MRSA菌株,SCCmecⅡ型15株(12.5%),SCCmecⅢ型96株(80%),SCCmec未定型9株(7.5%);120株MRSA菌株对β内酰胺类抗生素、喹诺酮类抗生素高度耐药;SCCmecⅡ型对克林霉素耐药性高(93.3%),而对四环素耐药性低(26.7%);SCCmecⅢ型对庆大霉素耐药高(91.7%);SCCmec未定型对复方磺胺甲噁唑耐药性高(77.8%)。不同SCCmec基因型MRSA对奎奴普汀、万古霉素、呋喃妥因100%敏感。结论目前医院的流行的MRSA多为SCCmecⅡ型和SCCmecⅢ型,各型对β-内酰胺类抗生素、喹诺酮类抗生素、氨基糖苷类、四环素类、大环内酯类均表现不同程度的耐药,今后应加强耐药基因的监测有效控制MRSA感染流行。Objective To investigate the SCCmec genotypes and drug resistance of methicillin resistant Staphylococcus aureus from ICU. Methods The SCCrnec genotyping was performed by multiplex PCR. The drug susceptibility of MRSA isolates was tested with AST GP67. Results Among 120 MRSA strains, 15 (12.5%) strains were SCCmec Ⅱ type, 96 (80%) strains were SCCmecⅢ type, 9(7.5%) strains were undifferentiated. All the 120 MRSA strains were highly resistant to the β-lactam antibiotics and quinolone antibiotics. SCCmec Ⅱ type were highly resistant to clindamycin(93.3%), while were lowly resistant to tetracycline(26.7%); SCCmecⅢ type were highly resistant to gentamicin(91.7%); undifferentiated strains were resistant to cotrimoxazole(77.8%). Different SCCmec genotypes of MRSA were sensitive to quinupristin, vancomycin and nitrofurantoin. Conclusion Currently SCCmec Ⅱ type and SCCmecⅢtype of MRSA were prevalence in hospital. Each SCCmec genotype of MRSA were showed different resistant to β-1actam antibiotics, quinolones, aminoglycosides, tetracyclines, macrolides. In the future we should further strengthen the monitoring of resistance genes and taken effective measures to control the infections of MRSA.
