维持性血液透析患者院内感染影响因素及其干预效果分析

摘　　要：【目的】探讨维持性血液透析患者院内感染的影响因素及其干预措施。【方法】选取维持性血液透析院内感染患者232例，分析维持性血液透析患者院内感染的影响因素，并实施相应干预措施。232例患者根据意愿分为观察组和对照组，分别采用不同的干预措施，比较两组干预后的疗效。【结果】维持性血液透析患者院内感染影响因素包括年龄、性别、吸烟史、高盐饮食、收入水平、长期置管、抑郁自评量表（SDS）评分、营养不良、侵入性操作、免疫功能低下。观察组干预后卡氏评分为（79．2±3．2）分，显著高于对照组（46．9±2．1）分（P〈0．05）；观察组躯体化、强迫症状、人际关系敏感、抑郁、敌对、恐怖、偏执及精神病性评分、抑郁评分均显著低于对照组，差异有统计学意义（P〈0．05）。【结论】维持性血液透析患者院内感染影响因素有很多，针对影响因素对患者实施临床干预，能提高患者生活质量，改善临床症状。[Objective]To investigate outcome of interventions on influence factors of nosocomlal miectlon of patients with continuous hemodialysis. [Methods]The influence factors of 232 patients with infection in our hospital were retrospectively analyzed, and corresponding intervention measures were performed. 120 patients were divided into experimental group and control group according to patients preference. Different intervention measures were carried out for these two groups, and the outcomes were compared between these two groups. [Results] Influence factors of nosocomial infection of patients with continuous hemodialysis include age, gender, history of smoking, high salt diet, income level, long-term catheter, SDS score, malnutrition, invasive operation and low immune function. Through targeted intervention for the experimental group, the survival quality of experimental group was better than the control group, and the control group had significantly higher number of deaths than in experimental group （ P 〈0.01）. The somatization, interpersonal sensitivity, hostility, paranoid disorder, terror~ psychosis and depression scores in experimental group was lower than in control group （ P 〈0.05）.[Conclusion] influence factors of nosocomial infection of patients with continuous hemodialysis are many, Intervention with these factors can improve the quality of patients＂ life and clinical symptomes.
