心力衰竭患者气阴两虚和阳气亏虚证客观特征分析

摘　　要：目的分析心力衰竭患者气阴两虚和阳气亏虚证的客观特征,为心衰中医临床本虚辨证提供客观依据。方法收集10所医院确诊为心力衰竭的患者340例,按照中医本虚辨证分为气阴两虚、阳气亏虚两组,对两组患者的性别、年龄、体重指数(BMI)、病程、吸烟史、饮酒史、原发疾病、心功能分级、脑钠钛(BNP)、左室射血分数(LVEF)、左室舒张末期内径、左室收缩末期内径等进行比较分析。结果两组心衰患者在性别、年龄、BMI、吸烟史、饮酒史、原发疾病、BNP水平等方面无统计学意义(P>0.05);在病程、心功能分级、LVEF有统计学意义(P<0.01);左室舒张末期内径、左室收缩末期内径比较差异有统计学意义(P<0.05或P<0.01)。结论心衰阳气亏虚证患者较气阴两虚证者病程长、心功能差。Objective By analysis the main objective indicator of heart failure（HF）patients with qi-yin asthenia syndrome and yang asthenia syndrome,providing the evidence of clinical differentiation of symptoms and signs for HF with deficiency in origin.Methods Three hundreds and forty patients who were diagnosed HF from ten hospitals of China.According to the differentiation of symptoms and signs of HF,the patients were divided into group qi-yin asthenia syndrome and group yang asthenia syndrome.The objective indicators were compared and analyzed between two groups,which included gender,age,body mass index（BMI）,disease course,smoking and drinking history,primary disease,New York Heart Association（NYHA）classification,brain natriuretic peptide（BNP）,left ventricular（LV）ejection fraction（LVEF）,LV end-diastolic dimension（LVEDD）,LV end-systolic dimension（LVESD）,etc.Results There was no statistically significant difference between two groups in gender,age,BMI,smoking and drinking history,primary disease and BNP（P 〉0.05）.Disease course,NYHA class and LVEF had statistically significant difference（P 〈0.01）.LVEDD and LVESD had statistically difference between two groups（P 〈0.05）.Conclusion Patients with yang asthenia syndrome have longer course and worse cardiac function than patients with qi-yin asthenia syndrome.
