稳心颗粒对室性早搏患者焦虑抑郁情绪及生活质量的影响

摘　　要：目的探讨稳心颗粒对室性早搏的治疗疗效、安全性及对患者焦虑、抑郁情绪和生活质量的影响。方法选择2011年1月—2011年12月在武汉大学人民医院心内科门诊及住院部就诊诊断为室性早搏的患者38例。所有入选患者停用其他抗心律失常药物至少5个半衰期,服用稳心颗粒,1日3次,1次1袋(9g),疗程为半年。入选患者首日均行生命体征、24h动态心电图(Holter)、血常规、肝肾功能等检查,完成焦虑自评量表(SAS)、抑郁自评量表(SDS)和SF-36生活质量量表,前1月内每周随访一次,观察有无药物不良反应,1月后再次行生命体征、Holter、血常规与肝肾功能等检查;半年后随访患者完成SAS、SDS和SF-36量表。结果治疗前后患者心率无明显变化(P>0.05),但治疗后早搏数较治疗前明显减少(P<0.05),治疗后动态心电图及症状疗效均明显。治疗后焦虑抑郁评分较治疗前明显减少(P<0.05)。治疗后SF-36量表8个维度生理功能、生理职能、躯体疼痛、一般健康状况、活力、社会功能、情感职能和精神健康评分较治疗前显著增加(P<0.05)。治疗后血常规、肝肾功能、血脂、电解质、凝血系列各指标与用药前无明显变化(P>0.05);用药过程中及用药后均无明显不良反应出现。结论稳心颗粒可降低焦虑抑郁评分,具有改善室性早搏患者焦虑和抑郁情绪作用。Objective To evaluate the effects and the safety of Wenxin granule（WXG）for treatment of ventricular premature beats（VPBs）,and observed the influence of it on anxiety,depression and quality of life in these patients.Methods Thirty-eight patients with VPBs from the Wuhan University People’s Hospital of cardiology outpatient and inpatient departments from January 2011 to December2011 were selected.All of the cases stopped currently taking other anti-arrhythmic drugs at least five half-life.All the study subjects taked WXG9 g each time,3 times a day for half year.All patients were monitored by holter and observed signs of life,clinical symptoms,the blood routine,liver and renal function,cholesterol and blood coagulation series before and after one month treatment respectively.The anxiety,depression and quality of life were evaluated by Self-Rating Anxiety Scale（SAS）,Self-Rating Depression Scale（SDS）and 36-item Short Form Health Survey（SF-36）before and after treatment respectively.Results There was no changes in the average heart rates between before and after treatment in patients（P 〉0.05）.The number of ventricular premature beats was significantly reduced after treatment（P 〈0.05）.These patients existed remarkable holter efficacy and clinical symptoms efficacy.The scores of SAS and SDS were significantly reduced（P 〈0.05）.The scores of SF-36 scales physical functioning（PF）,role-physical（RP）,bodily pain（BP）,general health（GH）,vitality（VT）,social-functioning（SF）,role-emotional（RE）,mental health（MH）were increased significantly after treatment.There was no adverse reactions.Conclusion WXG can reduce anxiety and depression,and improve the quality of life in patients with VPBs.
