摘  要:目的:探讨能谱CT使用低管电压和低浓度对比剂结合高清共轭采集技术对冠状动脉CTA检查的图像质量和辐射剂量的影响。方法:选择临床拟诊冠心病及支架术后行冠状动脉CTA复查的患者152例,随机分成A、B两组,各76例。A组:使用管电压100kVp,对比剂浓度270mg I/mL,128层40mm的高清(HD)共轭容积数据采集。B组:使用管电压120kVp,对比剂浓度为320mg I/mL,采用128层40mm的标准(STD)共轭容积数据采集。两组其它扫描参数不变。对两组图像质量进行评分,并记录和计算图像主动脉根部对比噪声比(CNR)、冠状动脉各段CT值的均值及冠状动脉显示节段数,计算两组的辐射剂量(ED)和人均碘摄入量,对两组间上述指标进行统计学分析。结果:A、B组图像质量评分分别为(3.52±0.48)和(3.12±0.56)分,且差异具有统计学意义(t=5.01,P<0.05);A组图像CNR高于B组(t=4.18,P<0.05);两组冠状动脉各段CT值均无统计学差异(P均>0.05),均可满足诊断需求;A组冠状动脉可评价节段数为1064,B组为1032;A、B组的ED值分别为(2.72±0.58)和(3.59±0.62)mSv,差异有统计学意义(t=9.66,P<0.05);A组人均碘摄入量[(234.42±10.23)mg I/kg]低于B组[(294.32±12.07)mg I/kg,t=33.09,P<0.001]。结论:能谱CT冠状动脉成像中,使用100kVp和低浓度等渗对比剂结合高清共轭采集技术,可有效降低辐射剂量,减少人体碘摄入量,同时提高整体图像质量,对冠状动脉狭窄病变的诊断提供了一定的帮助。Objective:To study the influence of spectrum CT using low tube voltage and low concentration of contrast agent combined with HD conjugate acquisition technology on the coronary CTA image quality and radiation dose.Methods:152patients with suspected diagnosis of coronary heart disease and undergoing follow-up coronary CTA examination after stent operation were recruited and randomly divided into two groups,with 76 cases in each group.For Group A,100 kVp tube voltage,concentration of contrast medium 270 mg I/mL,128 slices 40mm of high-definition(HD)conjugate volume data acquisition were adopted.For Group B,tube voltage 120 kVp,contrast medium concentration of 320 mg I/mL,128slice40 mm of standard(STD)conjugate volume data acquisition were used.Other scanning parameters remained unchanged.The subjective image quality was evaluated,the aortic root image contrast-to-noise ratio(CNR),the mean CT value of each segment of coronary artery and coronary CT values and the number of segments were recorded,radiation doses(ED)of the two groups were compared,the mean and standard deviations were calculated,and statistical analysis was made.Results:The two sets of image quality score were(3.52±0.48)and(3.12±0.56)points,the difference was statistically significant(t=5.01,P〈0.05).The image contrast-to-noise ratio in group A was higher than that in group B(t=4.18,P〈0.05).As for the CT values of each segment of coronary artery,there was no statistical difference in both groups.All could meet the demand of diagnosis(P〉0.05).In Group A,coronary artery evaluation segment number was 1064,it was 1032 in group B.ED value of the two groups of images were respectively(2.72±0.58)and(3.59±0.62)mSv,having statistically significant difference(t=9.66,P〈0.05).Contrast medium iodine intake per capita of group A [(234.42±10.23)mg I/kg]was lower than that of group B [(294.32±12.07)mg I/kg,t=33.09,P〈0.001].Conclusion:Spectrum CT coronary angiography using 100 kVp and low c
