腺苷注射液治疗肺挫伤的作用机制

摘  要:目的探讨腺苷注射液对撞击所致肺挫伤的治疗作用机制。方法选取30只健康日本大耳兔,随机分为对照组和腺苷注射液治疗组,各15只。用BIM-Ⅱ型生物撞击机制备兔肺挫伤模型。分别于撞击前10min、撞击后1h、3h、5h、7h采集颈总动脉血栓测丙二醛(MDA)、白介素.10(IL-10)的浓度;于伤后7h取下右肺,测定肺组织中髓过氧化物酶(MPO)含量;测定支气管肺泡灌洗液中的肿瘤坏死因子-α(TNF-α)含量和肺泡表面活性物质(PS)水平;解剖观察肺组织和病理改变。结果对照组血浆MDA浓度、肺组织中MPO含量、支气管肺泡灌洗液中TNF-α浓度、NE活性较腺苷组显著升高,而腺苷组血浆IL-10浓度和支气管肺泡灌洗液中总磷脂TPL含量较对照组明显高,同时腺苷治疗组肺组织的病理损害明显较对照组减轻。结论腺苷注射液对肺挫伤有治疗作用,其机制可能与调节肺挫伤早期炎性因子浓度有关。Objective To investigate the mechanism of adenosine injection in the treatment of lung contusion following chest impact. Methods Thirty healthy Japan big-ear rabbits were randomly assigned to the lung contusion control group ( control group, n=15 ) and adenosine injection group ( adenosine group, n=15 ) .The rabbits with lung contusion were induced by BIM-Ⅱ Horizontal Bioimpact Machine. Blood samples were taken from common carotid artery of both groups to measure the levels of plasma malondialdehyde ( MDA ) and IL-10 at 10 minutes before impact andl, 3, 5, 7 hours after impact.Rabbits were dissected to observe the gross and micmpathological changes of the lung at 7 hours after impact, the lung tissue myeloperoxidase ( MPO ) acticity and tumor necrosis factor-A ( TNF- α ) and elastase ( NE ) contents in bronchalveor lavage fluid ( BALF ) were obtained. Results The control group showed a distinct increasing of plasma MDA and the lung tissue MPO acticity and TNF- α and NE contents in BALF, The morphological observation revealed severe lung tissue injury as well. The adenosine group showed a distinict inreasing of plasma IL-10 and TPL contents in BALE Conclusion Adenosine injection has therapeutic effect on lung contusion by regulating inflammatory factor levels in the early period after lung contusion.
