摘 要:目的探讨糖尿病肾病患者尿蛋白对糖化白蛋白(GA)的影响。方法糖尿病肾病患者122例,其中肾病综合征患者47例,非肾病综合征患者75例,比较各组患者尿蛋白定量与GA的相关性,应用多因素回归分析影响糖尿病肾病患者GA的因素。结果GA与24h尿蛋白定量(UP)在肾病综合征组明显相关(r=-0.382,P〈0.05),在非肾病综合征组无相关性;肾病综合征组GA/糖化血红蛋白(HbAlc)水平显著低于非肾病综合征组(P〈0.05),多因素回归分析显示在肾病综合征组,仅空腹血糖(FBG)是影响GA的因素。结论肾病综合征水平的尿蛋白可以降低GA的数值,这种改变可独立于高血糖对GA的影响之外,故在使用糖化血红蛋白评估糖尿病患者血糖控制情况时,亦需考虑尿蛋白的影响。Objective To test the influence of urine protein on glycated albumin in diabetic nephrology patients. Methods A total of 122 patients were enrolled in this study, of whom 47 were nephrotic-range proteinuria.The correlation between UP and GA were examined. Results The GA and GA to HbA1 c ratio was significantly lower in nephrotic-range proteinuria than non-nephrotic-range proteinuria, while no significant difference in HbAlc or glucose levels was found.There was a signifcantly correlation between UP and GA in nephrotic-range proteinuria ( r=-0.382 , P〈0.001 ) , however no significant correlation was found in cases with non-nephrotic-range proteinuria. Conclusion Nephrotic-range proteinuria decreases GA values independent of the glycemic state, while nonnephrotic range proteinuria has no significant influence on GA values in CKD patients with diabetes.
