摘 要:目的 探讨视网膜光凝对增殖性糖尿病视网膜病变(PDR)新生血管膜粘连程度的影响.方法 对2014年1月至2015年1月在河南省立眼科医院就诊的PDR患者73例80只眼,其中光凝组和非光凝组各40只眼.术前B超检查眼后段情况,玻璃体手术中观察新生血管膜粘连特征,分析光凝组和非光凝组新生血管膜程度的差异.结果 激光组视网膜新生血管粘连范围>1 PD的眼数为18只眼(45%),非激光组为29只眼(72.5%),两组差异具有统计学意义(x2=6.241,P =0.012).两组在颞上、颞下、鼻上和鼻下部位发生新生血管的眼数差异无统计学意义.激光组伴有牵拉性视网膜脱离的眼数为19只眼(47.5%),非激光组为24只眼(60%),两组差异无统计学意义(P>0.05).结论 视网膜光凝可以减轻增殖性糖尿病视网膜病变新生血管膜粘连程度,降低手术难度。Objective To identify the effect of retinal photocoagulation on the degree of adhesion between the retinal surface and the fibrovascular tissuein proliferative diabetic retinopathy(PDR).Methods Pars planavitrectomy was performed for the 73 cases (80 eyes) with PDRin type 2 diabetes.Theseeyes were assigned to the retinal photocoagulationand noretinal photocoagulation groups(40 eyes,respectively).All cases had B-scan examination.The morphologic characteristics of fibrovasculartissueinneovascularization elsewhere(NVE)was identified during the surgery.The differences of adhesiondegree between the prior retinal photocoagulation group and no priorretinal photocoagulation groupwere compared.Results Eighteen eyes (45%) and 29 eyes (72.5%)with adhesionalfibrovascular tissue of 〉 1PD area was observedin the retinal photocoagulation andnoretinal photocoagulation groups, respectively;there was a significant difference(P 〈0.05) on both groups.Adhesionalfibrovascular tissuewas observedin the supertemporal, Infratemporal,supemasal and intranasal quadrant,respectively;there wasno significant differencein both groups.Nineteen eyes (47.5%) and 24 eyes (60%) with tractional retinal detachment was observedin the retinal photocoagulationand no retinal photocoagulation groups, respectively;there was no significant difference(P 〉0.05) on both groups.Conclusions Retinal photocoagulation on the fibrovascular proliferations in PDR is effective at preventing PDR development, reducingthe degree of fibrovascularadhesionandsimplifyingoperation in difficulty.
