CT三维重建在胆道金属支架植入术前评估中的应用价值

摘　　要：目的探讨CT三维重建用于胆道金属支架植入术前的评估价值。方法选择2013年12月至2014年8月我院收治的梗阻性黄疸患者22例,在术前通过CT三维重建进行评估,实施胆道金属支架植入术治疗,观察患者术前及术后临床症状及临床指标变化。结果本组22例患者经CT三维重建术前评估,实施胆道金属支架植入术治疗梗阻性黄疸全部获得成功。术后3 d患者的临床症状均明显得到好转。术后7 d患者的血清总胆红素值、血清直接胆红素值、丙氨酸氨基转移酶、胆总管内径均明显下降(P<0.05)。22例患者全部没有出现明显的并发症。结论在术前运用CT三维重建给予评估,可对胆道金属支架植入的可行性、胆道金属支架直径及长度选择、术中情况进行准确预判,明显提升胆道金属支架植入术的治疗效果,没有明显的并发症,值得应用及推广。Objective To investigate the value of CT three dimensional reconstruction in preoperative evaluation of biliary metal stent implantation. Methods 22 patients with obstructive jaundice in 2013 December to 2014 August in our hospital were evaluated in patients before operation by CT three-dimensional reconstruction,the implementation of biliary metal stent implantation in treatment,to observe the changes of clinical symptoms and clinical indexes of patients before and after surgery. Results To evaluate the preoperative CT 3D reconstruction of 22 patients in this group,the implementation of biliary metal stent implantation is all successful treatment of obstructive jaundice. Postoperative clinical symptoms in 3D patients were significantly improved. The postoperative serum total bilirubin in patients with 7d and serum direct bilirubin,alanine aminotransferase,the diameter of common bile duct were significantly decreased（ P〈0. 05）. All 22 cases of patients with no obvious complications. Conclusions The use of CT three-dimensional reconstruction in preoperative assessment can be given feasibility of biliary metal stent implantation of biliary metal stent diameter and length selection,intraoperative accurately predict,significantly enhance the biliary tract Stand Implantation of metal treatment,no obvious complications,worthy of application and promotion.
