摘  要:目的探讨脑白质病变与血浆过氧化物酶体增殖物激活受体-γ(PPAR-γ)及血浆维生素B12水平的相关性。方法选择CT、MRI证实存在脑白质病变、脑梗死及未发现脑白质病变患者140例,测定并比较其血浆PPAR-γ与维生素B12水平。结果存在脑梗死或脑白质病变患者血浆PPAR-γ浓度高于头颅MRI或SCT未发现脑白质病变的患者,差异有统计学意义(P<0.05)。单纯脑白质病变患者维生素B12浓度高于脑梗死合并脑白质病变或无合并脑白质病变患者,差异有统计学意义(P<0.05)。结论脑白质病变患者血浆PPAR-γ浓度升高,维生素B12浓度的降低与脑血管病变的严重程度相关,两者不仅与大血管病变关系密切,与小血管病变引起的脑白质病变也有密切关系。Objective To investigate the relevance between white matter lesions levels of plasma PPAR-γ and plasma vitamin B12.Methods 140 cases of white matter lesions and cerebral infarction proved by CT and MRI,as well as cases without white matter lesions were selected.Plasma PPAR-γ and vitamin B12 levels were measured and compared between groups.Results In comparison of plasma PPAR-γ levels,patients with cerebral infarction or white matter lesions had higher plasma PPAR-γ concentration than that of patients with normal head MRI or SCT,with a statistically significant difference( P 0.05).In comparison of levels of vitamin B12,the vitamin B12 concentration of patients with white matter lesions only was higher than that of patients with combined cerebral infarction and white matter lesions or with cerebral infarction alone,with a statistically significant difference( P 0.05).Conclusion Increased levels of plasma PPAR-γ and decreased levels of plasma vitamin B12 are related to the severity of cerebrovascular lesions in patients with white matter lesions.Both of them are not only closely related with macroangiopathy but with white matter lesions resulting from small vessel vasculopathy.
