自体干细胞移植治疗晚期肝硬化病人的疗效及围术期护理

摘　　要：[目的]总结自体干细胞移植治疗晚期肝硬化病人的疗效及围术期护理。[方法]对100例内科保守治疗无效的晚期肝硬化病人采用自体外周血干细胞移植,术前采取针对性心理干预,做好术后康复指导,观察药物不良反应,术后严密观察生命体征变化,预防并发症发生。随访1年后观察病人一般情况及肝功能情况。[结果]100例病人术后1年存活率100%,其中52例大量腹水病人术后腹水逐渐减少直到消失,4例术后3个月出现上消化道出血,3例出现Ⅰ度~Ⅱ度肝性脑病。移植术后6个月、12个月肝脏合成功能、肝脏储备功能、肝弹性测定均有显著改善,与移植前相比差异有统计学意义（P〈0.05）。[结论]自体外周血干细胞移植可以有效地改善晚期肝硬化病人的肝脏功能、肝组织形态学,提高病人的生活质量,围术期护理可促进病人康复,提高治疗效果。Objective：To summarize the curative effect and perioperative care of advanced cirrhosis patients treated with autologous stem cells transplantation.Methods：A total of 100 advanced cirrhosis patients who had failure conservative therapy by internal medicine were conducted with autologous peripheral blood stem cells transplantation for advanced cirrhosis.Preoperative targeted psychological intervention,targeted psychological intervention was taken for them,and postoperative rehabilitation guidance was did well,the drug adverse reactions were observed,the vital signs were observed closely and to prevent the complications.Results：One-year survival rate was 100%in 100 patients,the ascites was gradually reduced and disappeared in 52 patients with massive ascites.4patients underwent upper gastrointestinal bleeding at postoperative 3months and 3patients developed with Ⅰ-Ⅱ degree of hepatic encephalopathy.After transplantation,the liver synthetic function,liver reserve function and liver elasticity determination were significantly improved（P0.05）compared with that before transplantation.Conclusion：Autologous peripheral blood stem cells transplantation could improve the liver function and liver tissue morphology of advanced cirrhosis patients and their quality of life.Perioperative holistic nursing can promote the rehabilitation of patients and improve the curative effect.
