肥胖腹股沟疝患者行前入路腹膜前无张力修补术78例报道

摘　　要：目的总结肥胖腹股沟疝患者行前入路腹膜前无张力修补术的经验。方法选择2010年7月至2014年7月,昆山市第六人民医院接受前入路腹膜前无张力疝修补术的肥胖腹股沟疝患者78例,回顾性分析临床及随访资料,观察患者的手术时间,术后恢复自主活动时间,住院时间,术后并发症发生情况和复发情况等,并与相关文献数据对比分析。结果本组78例患者中,77例成功完成前入路腹膜前无张力修补术。手术时间(65±13)min,术后恢复自主活动平均时间(10.0±2.0)h,平均住院时间(7.0±2.0)d。本组患者与文献组1患者比较,在手术时间、切口疼痛方面差异有统计学意义,本组患者手术时间较长,但发生疼痛较少。与文献组2患者比较,在平均住院时间和术后恢复自主活动时间方面差异有统计学意义,本组患者在上述两方面用时较短。结论应用前入路腹膜前无张力疝修补术治疗伴有肥胖症的腹股沟疝患者效果良好,有在基层医院推广和临床应用的价值。Objective To introduce the experience of anterior preperitoneal tension-free hernia repair for inguinal hernia in patients with obesity. Methods A retrospective analysis of 78 cases with obesity who received anterior preperitoneal tension-free hernia repair were conducted to investigate the operating time,postoperative recovery activity time,hospitalization time,postoperative complications and recurrence,etc. All of the indicators above would be compared with the data in the literature. All the cases in the study came from the department of general surgery,the Sixth People＇s Hospital of Kunshan City,from July 2010 to July 2014. Results The procedures in 77 out of 78 patients were successful. The operating time,postoperative recovery activity time and hospitalization time were（ 65 ± 13） minutes,（ 10. 0 ±2. 0） hours and（ 7. 0 ± 2. 0） days,respectively. Compared with literature group 1,the results were significantly different in terms of operation time and pain of incision. The time of the surgery for this group of patients is longer,but the pain is less. Compared with literature group 2,the length of stay and postoperative recovery activity time were shorter in this series of patients. Conclusion Anterior preperitoneal tension-free inguinal hernia repair is an effective procedure for patients with obesity. It is worthy to be introduced to primary hospitals widely.
