摘　　要：目的剖宫产术中应用可吸收止血纱布后,由于可吸收止血纱布吸收不完全形成盆腔异物肉芽肿,探讨其超声声像图特征及诊断价值。方法收集2011年7月—2014年7月济宁医学院附属医院彩色多普勒超声发现盆腔包块,且术后病理证实为盆腔异物肉芽肿的患者27例,回顾性分析其声像图特征。结果 27例患者均有剖宫产手术并放置可吸收止血纱布史,彩色多普勒超声发现盆腔包块,包块与子宫前壁下段关系密切、分界不清,包块均为单发,均可见包膜样结构,形态多为圆形、椭圆形,内部回声不均匀。其中混合性回声21例(77.8%),不均质低回声6例(22.2%);未探及明显血流信号22例(81.5%)。结论剖宫产术后可吸收止血纱布未完全吸收形成盆腔异物肉芽肿,超声表现有一定的特征性,能够给临床提供有价值的诊断信息。Objective To investigate the ultrasonographic features and diagnostic value of foreign body granuloma in pelvic cavity caused by incomplete absorption of gauze after applying absorbable hemostatic gauze in cesarean section. Methods Collected the data of 27 patients who were detected with pelvic mass by color doppler ultrasound and were confirmed as foreign body granuloma by postoperative pathology in the Affiliated Hospital of Jining Medical College from July 2011 to July 2014. We made a retrospective analysis of the ultrasonographic features. Results All the 27 patients had placement of absorbable hemostatic gauze in cesarean section and were detected with pelvic masses by color doppler ultrasound. The pelvic masses had close association with the anterior wall of the uterus,but with no clear boundary between them. The masses were all solitary with coated structure and were shaped as round or oval with inhomogeneous internal echoes. Among them,21（ 77. 8%） had mixed echo,6（ 22. 2%） had inhomogeneous low echo,and 22（ 81. 5%） had no obvious blood flow signals. Conclusion Foreign body granuloma in pelvic cavity caused by incomplete absorption of gauze after applying absorbable hemostatic gauze in cesarean section has characteristic ultrasonography,which can provide valuable references for clinical diagnosis.
