输尿管镜钬激光碎石治疗体外震波碎石失败后输尿管结石35例临床观察

摘　　要：目的探讨输尿管镜钬激光碎石治疗体外震波碎石(ESWL)失败输尿管结石的临床疗效。方法2011年5月—2013年11月35例体外震波碎石失败后的输尿管结石患者,采用WOLF F8/9.8 10°输尿管镜,以钬激光碎石,碎石功率30W。结果 35例中一次性碎石成功32例,碎石成功率为91.4%;2例术中结石上漂,术后行体外震波碎石成功,1例输尿管结石因输尿管狭窄进镜失败改行输尿管切开取石术。手术时间20 min^125 min,平均时间40 min。术后常规留置双J管2周~4周。结论输尿管镜钬激光碎石术治疗ESWL失败后输尿管结石疗效确切,是安全、有效的方法。ObjectiveTo investigate the ureteroscopic Holmium：YAG laser lithotripsy in the treatmet of ureteral stones after failure of extracorporeal shock wave lithotripsy（ESWL）.MethodsFrom 2011 May to 2013 November,35 patients of the ureteral stones,after the failure of extracorporeal shock wave lithotripsy（ESWL）,using WOLF F8/9.8 10 ureteroscopy with Holmium：YAG laser lithotripsy,power 30W.ResultsThe overall stone-free rate was 91.4%（32/35）,2 patients were intraoperatively switched to ESWL for crashing stones.1 patient with ureteral stricture after failure of ureteroscopy received ureterolithotomy. The operation time was 20-125 minutes（average 40 minutes）.The D-J stent was placed after 2-4 weeks.ConclusionUreteroscopy Holmium：YAG laser lithotripsy is a effective and safe therapy for treating the ureteral stones after failure of ESWL.
