卡前列素氨丁三醇注射液联合Foleys导尿管宫腔填塞在宫缩乏力性产后出血中的临床应用

摘　　要：目的探讨卡前列素氨丁三醇注射液联合Foleys导尿管宫腔填塞在宫缩乏力性产后出血中的应用价值。方法选取2013年3月~2015年3月我院产科收治的90例宫缩乏力性产后出血患者作为研究对象,采用随机数字表法将其分为研究组和对照组,每组45例,研究组采用卡前列素氨丁三醇注射液联合Foleys导尿管宫腔填塞治疗,对照组采用常规止血治疗,观察比较两组患者术中、产后出血量,血红蛋白改变情况,止血、住院时间及治疗有效率。结果研究组术中、产后2 h、产后24 h出血量均少于对照组（P〈0.05）;研究组血红蛋白产前、产后无明显下降,优于对照组（P〈0.05）;研究组止血时间、住院时间分别为（8.6±1.3）min、（22.6±1.3）d,均短于对照组的（22.5±3.2）min、（7.8±1.7）d（P〈0.05）;研究组治疗有效率为93.3%,明显高于对照组的75.6%（P〈0.05）。结论卡前列素氨丁三醇注射液联合Foleys导尿管宫腔填塞治疗宫缩乏力性产后出血效果确切,可有效减少患者术后出血量,缩短住院时间,提高治疗有效率。Objective To explore the application value of carboprost trometamole injection combined with Foleys catheter for uterine packing in postpartum hemorrhage caused by uterine inertia. Methods From March 2013 to March2015,90 cases of postpartum hemorrhage due to uterine inertia admitted into our obstetric department of our hospital were selected as research objects.By random number table,they were evenly divided into research group and control group in random.In the research group,carboprost trometamole injection combined with Foleys catheter for uterine packing was applied,while in the control group,conventional hemostasis was used.The intraoperative and postpartum amount of bleeding,changes of hemoglobin,stanching bleeding time,hospital stay,and effective rate of treatment were comprehensively compared between two groups. Results In the research group,the amount of bleeding during surgery,2 h and 24 h after delivery was less than that of control group respectively（P0.05）.There was no obvious decrease in hemoglobin before and after delivery,but the change of hemoglobin in observation group was superior to that in control group（P0.05）.The time of stanching bleeding and hospital stay in research group was（8.6±1.3） min and（22.6±1.3）d respectively,shorter than that of control group（22.5±3.2） min and（7.8±1.7） d respectively（P0.05）.The effective rate of treatment in research group was 93.3%,much higher than that in control group accounting for 75.6%（P0.05）. Conclusion Carboprost trometamole injection combined with Foleys catheter for uterine packing can obtain a definite effect on treating postpartum hemorrhage due to uterine inertia,which effectively reduce patient′s postoperative amount of bleeding,shorten hospital stay,and improve effective rate of treatment.
