186例Ⅰ类手术切口围术期预防用药情况分析

摘　　要：目的了解本院Ⅰ类切口手术预防使用抗菌药物的情况,为进一步规范其使用提供科学依据。方法回顾性调查本院2014年7~11月的186例Ⅰ类切口手术病例,对抗菌药物选用品种、联合用药情况、预防用药时机、预防用药起止时间等进行统计分析。结果 186例Ⅰ类切口手术中,预防使用抗菌药物26例（14.0%）。预防使用抗菌药物病例中,不合理使用达73.1%（19/26）,主要表现在药物选择不合理、用药时机选择不恰当及术后使用时间偏长等方面。结论本院Ⅰ类切口手术抗菌药物预防使用率较为规范,但在药物选择及用药时机方面存在较多问题,术后用药时间偏长,应进一步规范临床围术期抗菌药物的使用与管理,提高本院临床对抗菌药物的合理应用。Objective To understand the situation of preventive usage of antibacterial agents on type Ⅰ surgical incision in our hospital to provide a scientific basis for further regulating the usage. Methods 186 medical records with type Ⅰ surgical incision from July 2014 to November were retrospectively investigated.The variety of antibacterial agents,drug combination,timing of preventive administration and time to start and terminate prevention drug usage were statistically analyzed. Results Among 186 medical records with type Ⅰ surgical incision,there were 26 cases applied with preventive administration of antibiotics,and usage rate was 14.0%.The irrational drug usage rate in preventive administration of antibiotics reached 73.1%（19/26）,which were reflected as irrational drug selection,inappropriate time to use drugs,and long time to use the drugs after surgery,etc.. Conclusion Preventive usage rate of antibiotics for type Ⅰsurgical incision in our hospital is relatively normalized,but there are still some problems existed like drug selection,administration occasion,and long time to use the drugs after surgery and so forth,which should be strengthened in application management of antibiotics during perioperative period in order to improve the rational usage level of antibiotics.
