摘 要:目的探讨子痫前期并发急性肾损伤(AKI)患者尿液中肾损伤分子-1(KIM-1)、β2-微球蛋白(β2-MG)的水平变化及其临床意义。方法选择2012年1月至2014年7月湖北医药学院附属襄阳医院收治的子痫前期并发AKI患者40例为观察组,另选择健康孕妇40例为对照组,分别于孕30、32、34、36周测定血肌酐(SCr)水平、尿液中KIM-1和β2-MG水平及尿量。结果观察组患者孕36周时SCr水平显著高于孕30周(P<0.01),对照组在孕30、32、34、36周时SCr水平两两比较差异均无统计学意义(P>0.05),观察组患者孕36周时SCr水平显著高于对照组(P<0.05)。观察组患者孕36周时尿量显著少于孕30周(P<0.01),对照组孕30、32、34、36周时尿量两两比较差异均无统计学意义(P>0.05),观察组患者36周时尿量显著少于对照组(P<0.05)。观察组患者孕32、34、36周时尿KIM-1水平显著高于孕30周(P<0.05,P<0.01),对照组孕30、32、34、36周时尿KIM-1水平两两比较差异均无统计学意义(P>0.05),观察组患者孕32、34、36周时尿KIM-1水平显著高于对照组(P<0.05)。观察组患者孕34、36周时尿β2-MG水平显著高于孕30周(P<0.05,P<0.01),对照组孕30、32、34、36周时尿β2-MG水平两两比较差异均无统计学意义(P>0.05),观察组孕34、36周时尿β2-MG水平显著高于对照组(P<0.05)。结论子痫前期并发AKI患者尿液中KIM-1、β2-MG水平改变早于SCr和尿量改变,在子痫前期孕妇产前监控中,尿KIM-1、β2-MG水平检测有利于早期诊断肾损伤。Objective To investigate the clinical significance of the changes of kidney injury molecules-1( KIM-1) andβ2-microglobulin( β2-MG) in urine of pregnant women with preeclampsia complicated with acute kidney injury( AKI). Methods Forty patients with preeclampsia complicated with AKI( observation group) and forty healthy pregnant women( control group) were selected in Xiangyang Hospital Affiliated to Hubei University of Medicine from January 2012 to July 2014. The levels of serum creatinine( SCr),KIM-1,β2-MG in urine and urine volume were detected at 30,32,34 and 36 weeks after pregnancy. Results The SCr level at 36 weeks after pregnancy was significantly higher than that at 30 weeks in observation group( P〈0. 01). There was no significant difference in SCr level among 30,32,34 and 36 weeks after pregnancy in control group( P〉0. 05). The SCr level at 36 weeks after pregnancy in observation group was significantly higher than that in control group( P〉0. 05). The urine volume at 36 weeks after pregnancy was significantly less than that at 30 weeks in observation group( P〈0. 01). There was no significant difference in urine volume among 30,32,34 and 36 weeks after pregnancy in control group( P〈0. 05). The urine volume at 36 weeks after pregnancy in observation group was significantly higher than that in control group( P〉0. 05). The level of KIM-1 in urine at 32,34 and 36 weeks was significantly higher than that at 30 weeks in observation group( P〈0. 05,P〈0. 01). There was no significant difference in the level of KIM-1 in urine among 30,32,34 and 36 weeks after pregnancy in control group( P〉0. 05). The level of KIM-1 in urine at 32,34 and 36 weeks after pregnancy in observation group was significantly higher than that in control group( P〈0. 05). The level of β2-MG in urine at 34 and 36 weeks after pregnancy was significantly higher than that at 30 weeks in observation group( P〈0. 05,P〈0. 01). There was no significant difference
