摘 要:目的研究脊髓背角单羧酸转运蛋白-2(monocarboxylatetransporter-2,MCT-2)在术后慢性疼痛中的作用。方法76只健康雄性SD大鼠,采用随机数字表法分为3组:假手术组(S0组)、模型组(S1组)和给药组(S2组)。S1组和S2组建立皮肤肌肉切口牵拉术(skin/muscleincisionandretraction,SMIR)动物模型,S0组仅切开而不进行SMIR操作。术后第10天,S2组鞘内注射溶解于10%二甲基亚砜(dimethylsulfoxide,DMSO)10μl中的d氰基4堀基肉桂酸(α-cyano-4-hydroxy-einhamate,4-CIN)100μmol,S1组仅鞘内注射10%DMSO10μl。各组在术前1d(-1d)及术后1、3、7、10、14、21、28d测定大鼠机械缩足反射阈值(pawwithdrawalmechanicalthreshold,PWMT)。So组与S2组于术后14d、S1组于-1d-28d各时间点分别取4只大鼠,处死后取术侧L4-5脊髓背角组织,采用Westernblot法测定MCT-2表达变化。结果各组术前PWMT差异无统计学意义(P〉0.05)。与S0组比较:S1组从术后3d[(10.7±3.4)g]起PWMT明显降低,且一直持续至28d[(10.6±2.5)g](P〈0.05),且术后1d-28d时脊髓背角MCT-2表达水平上调(P〈0.05);S2组PWMT在给药P后14d~28d,差异无统计学意义(P〉0.05)。与S1组比较,S0组与S2组在14d时脊髓背角MCT-2表达水平均下调(P〈0.05)。结论SMIR大鼠脊髓背角MCT-2表达上调,鞘内注射4-CIN可缓解SMIR所致的术后持续性疼痛,其机制可能与抑制脊髓背角MCT-2有关。Objective To investigate the role of monocarboxylate transporter-2 (MCT-2) in the spinal dorsal horn in rat's persistent postoperative pain. Methods Seventy six male SD rats were randomly divided into 3 groups: sham group (group So), skin/muscle incision and retraction (SMIR) model group (group S 1 ) and α -cyano-4-hydroxy-cinnamate (4-CIN) treatment group (group S2). The rats in the groups S1 and S2 were operated with the model of persistent postoperative pain induced by skin/muscle incision and retraction, the animals in the group So were treated as sham operated rats. Ten days after operation, the rats in the group D2 were intrathecally injected with 100 μmol 4-CIN dissolved in 10% dimethyl sulfoxide (DMSO) 10 μl, while the animals in the group S, were intrathecally injected with 10% DMSO 10 μl only. The paw withdrawal mechanical threshold (PWMT) was determined 1 d before operation and 1, 3, 7, 10, 14, 21 d and 28 d after operation. In the groups S0 and S2, 4 rats were killed on 14 d after operation. In the group S1, 4 rats were sacrificed at each time point. The L4-5 segments of spinal cord were removed to measure the expression of MCT-2 by Western blot analysis. Results The basic values of PWMT before operation were not statistically different among three groups (P〉0.05). The PWMT from 3 d[(10.7±3.4) g] to 28 d[(10.6±2.5) g] were significantly decreased in the group S1 compared with the group So (P〈0.05). However, the expression of MCT-2 in spinal dorsal horn from 1 d to 28 d were significantly higher in the group S2 than in the group So. The expression of MCT-2 in spinal dorsal horn on 14 d was significantly lower in the groups So and S2 than in the group S1. Conclusions The expression of MCT-2 in spinal dorsal horn is significantly up-regulated in a rat model of skin/muscle incision and retraction.
