H型高血压患者与非H型高血压患者肾脏早期损伤检测指标的比较

摘　　要：目的探讨H型高血压患者与非H型高血压患者肾脏早期损伤检测指标水平的变化。方法根据高血压患者空腹血清同型半胱氨酸(Hcy)水平,70例高血压患者Hcy≥10μmol/L者判为H型高血压,53例高血压患者Hcy<10μmol/L者判为单纯高血压,收集2组患者的清晨非应激、非运动状态下未污染的第1次中段尿15 ml送检,标本不加防腐剂,其中5 ml应用德国西门子特定蛋白分析仪BN2检测尿微量蛋白Ig G、α1-MG、MA及TRF,其中10 ml在希森美康AU1000作尿常规定量分析。结果 H型高血压组患者血清Hcy、Ig G、MA、α1-MG、TRF及尿红细胞、尿小圆上皮细胞、尿管型数等测定结果高于非H型高血压组,差异均具有统计学意义(P<0.05)。结论 H型高血压患者更易发生肾脏早期损伤,定期检测肾功能对发现肾脏早期损伤具有一定的临床价值。Objective To explore the differences on the detection indexes of the early renal injury between patients with H type hypertension and non- H- type hypertension. Methods According to the levels of fasting serum homocyteine（ Hcy） in patents with hypertension,70 patients were defined as H- type hypertension group（ Hcy≥10 μmol / L） and 53 patients as simple hypertension group（ Hcy 10 μmol / L）. 15 ml fresh first morning midstream urine was collected in two groups in the state of non- stress and non- exercise,without preservatives. Microalbuminuria Ig G,α1- MG,MA and TRF in the 5 ml midstream urine were measured by BN2 Siemens specific protein analyzer,and another 10 ml,for urine routine quantitative analysis,were detected by Sysmex AU1000. Results The examined levels of serum Hcy,Ig G,MA,α1- MG,TRF and the amount of red blood cells,small round cells,pipe models were higher in H- type hypertension group than that in the non- H- type hypertension group,and the differences were statistically significant（ P〈0. 05）. Conclusion H- type hypertension patients have higher risk for the occurrence of renal injury. Regular detection of the renal function has the certain clinical values for the early diagnosis of renal injury.
