血清VGEF、TNF-α的表达与慢性血栓栓塞性肺动脉高压大鼠肺血管重构的相关性研究

摘  要:目的:研究慢性血栓栓塞性肺动脉高压( chronic thrombo embolic pulmonary hypertension,CTEPH)炎症因子表达水平的动态变化以评估炎症反应与肺血管重构的关系. 方法:雄性 Wistar大鼠64 只,随机分为8 组,每组8 只:初始对照组( IC组)、栓塞3 天组、1 周组、2 周组、4 周组、8 周组、12 周组( D3、W1、W2、W4、W8、W12 )、终末对照组( EC组). 采血制备0. 5 mm左右大小的自体血栓,颈静脉注入27 ~30 个自体血栓,2 周后第2 次注入同样数目血栓,2 次术后肌注庆大霉素预防感染,全程腹腔注射氨甲环酸. 达各组实验目标日期后,测各组大鼠平均肺动脉压( mPAP)、肺动脉相对中膜厚度( PAMT)、管壁面积/管总面积( WA/TA)百分比,免疫组化检测血管内皮生长因子( VGEF)、肿瘤坏死因子( TNF-α)表达. 结果:第1 次栓塞后mPAP略有升高,与对照组比较无统计学差异(P〉0. 05),第2 次栓塞后随时间延长mPAP明显增高(均P〈0. 01);PAMT、WA/TA在4 周后随时间延长显著增高(4 周组P〈0. 05,8 周、12 周组P〈0. 01);不同时间组血清VGEF、TNF-α值较初始对照组和终末对照组均明显升高,P〈0. 01,差异有统计学意义,其中 W1 达峰值,其后有所下降,2 次栓塞后W4 再次达峰值,其后维持较高水平. 相关分析表明,VGEF与TNF-α及血管重构指标均呈正相关(P〈0. 01),VGEF与TNF-α表达呈正相关(P〈0. 01). 结论:VGEF及TNF-α均可能参与慢性血栓栓塞性肺动脉高压、肺血管重构的病理生理过程,炎症反应是慢性肺动脉高压形成重要机制之一.To study chronic thromboembolic pulmonary hypertension ( chronic thrombo embolic pulmonary hypertension, CTEPH) Changes in the expression levels of inflammatory cytokines in order to assess the relationship between pulmonary vascular remode-ling inflammatory response. Methods:64 male Wistar rats were randomly divided into 8 groups of 8: initial control group ( IC group) , embolism 3 d group, one week group, 2 weeks, 4 weeks, 8 weeks group, 12 weeks group (D3, W1, W2, W4, W8, W12), the terminal group ( EC group). Preparation of blood of about 0. 5 mm size autologous thrombus, jugular vein injection of 27 to 30 autologous thrombus, the second injection the same number thrombosis two weeks after two intramuscular gentamicin prevention of infection after sur-gery, throughout the intraperitoneal injection of tranexamic acid. Up to the target date after each set of experiments, the rats measured mean pulmonary arterial pressure ( mPAP) , pulmonary relative thickness ( PAMT) , wall area / total area ( WA / TA) percentage immu-nohistochemistry vascular endothelial growth factor ( VGEF) , tumor necrosis factor ( TNF-α) expression. Results:After the first embo-lization mPAP increased slightly, no significant difference (P〉 0. 05) with the control group, after the second embolization mPAP signifi-cantly increased with time (all P 〈0. 01);PAMT , WA / TA in four weeks after the extension was significantly increased over time (4 weeks group P 〈0. 05,8 week, 12 weeks group P 〈0. 01);serum at different times VGEF, TNF-αthan the initial value of the control group and terminal control group were significantly higher, P 〈0. 01, the difference was statistically significant, which reached its peak W1, then declined again after the Second embolization W4 peaked, then maintain a high level. Correlation analysis showed that, VGEF positively correlated with TNF-α and remodeling index showed (P 〈0. 01), VGEF TNF-αexpression was positively correlated (P 〈0. O1)
