经皮低频穴位电刺激对脑卒中后抑郁事件相关电位的观察

摘　　要：目的:探讨经皮低频穴位电刺激对脑卒中后抑郁事件相关电位(P300)的影响。方法:选取脑卒中后抑郁患者40例,随机分为治疗组20例,经皮低频穴位电刺激治疗;对照组20例,只进行康复宣教。治疗组和对照组均进行康复宣教,每周3次,共2周;治疗前、后分别用汉密尔顿抑郁量表(Hamilton,HAMD)、P300来观察组内、组间差异。结果:治疗组与对照组治疗后均较治疗前组内及组间HAMD评分明显降低,P300潜伏期(缩短)、波幅(增大)差异均具显著性意义(P<0.05),且治疗组改善幅度较对照组明显。结论:低频穴位电刺激、康复宣教两种方法对脑卒中后抑郁都有改善,低频穴位电刺激结合康复宣教疗效更好,P300评定相对更客观。但样本量小,疗效和机制需要进一步研究。Objective： To study the effects of percutaneous low- frequency electrical acupoint stimulation on depression after stroke event related potential（ P300）. Methods： 40 cases of patients with depression after stroke were selected and randomly divided into a treatment group of 20 cases with low frequency electrical acupoint stimulation and a control group of 20 cases with only rehabilitation education. The low- frequency electrical acupoint stimulation group and the control group were given rehabilitation education,three times a week,a total of 2 weeks; before and after using Hamilton Depression Scale（ Hamilton,HAMD）,P300 was used to observe the differences within and between groups. Results： After the treatment in low- frequency acupuncture group and the control group HAMD scores were significantly decreased,and the latency of P300 amplitude（ shortening） differences was significant（ P〈0. 05）; the treatment group was superior to the control group.Conclusion： Low frequency electric stimulation of acupuncture points and rehabilitation education for post stroke depression have effectiveness,and the low- frequency electrical acupoint stimulation combined with rehabilitation education is better; but because of the small sample size,further studies are needed.
