分期穴位埋线治疗原发性痛经的临床疗效观察

摘　　要：目的：观察分期穴位埋线治疗原发性痛经的近期和远期疗效。方法：将60例原发性痛经患者随机分成埋线组和对照组各30例。埋线组于经前期（月经来潮前5-10天）和经后期（月经干净第3-7天）分别埋线1次,经前期以关元、十七椎、子宫、次髎为主,经后期以肝俞、脾俞、肾俞、三阴交为主;对照组给予口服布洛芬缓释胶囊,月经来潮第1天开始口服,每次0.3 g,每天两次,至疼痛缓解。1个月经周期为1个疗程,连续治疗3个疗程后观察近期疗效,并随后停止治疗满3个月经周期时,随访远期疗效。采用痛经积分进行疗效评定。结果：埋线组治疗后痛经积分与对照组相比,差异具有非常显著性意义（P〈0.01）;埋线组近期和远期疗效与对照组相比,差异具有统计学意义（P〈0.05、P〈0.01）。结论：穴位埋线法的近期和远期疗效均优于口服西药布洛芬缓释胶囊,且穴位埋线法经济、安全、疗效持久稳定。Objective： To observe the short- term and long- term efficacies on primary dysmenorrhea treated by staging acupoint catgut embedment therapy. Methods： Sixty cases of primary dysmenorrheal were randomly assigned into an embedment therapy group and a control group,with 30 cases in each one. In the embedment therapy group,the embedment therapy was applied twice during the menstrual cycle,one treatment for 5 - 10 days before menstruation and one treatment for 3 - 7 days after menstrual clean. Guanyuan,Shiqizhui,Zigong and Ciliao were the main acupoints in the treatment for 5 - 10 days before menstruation. Ganshu,Pishu,Shenshu and Sanyinjiao were the main acupoints in the treatment for 3 - 7 days after menstrual clean. In the control group,fenbid was oraled,0. 3 g each time,twice a day,starting the first day of menstruation till pain was relieved. The treatment of one menstrual cycle was one session. The continuous treatment of 3 menstrual cycles was required. The short- term and long- term efficacies were evaluated at the end of the 3rd cycle and in 3months after the treatment. The dysmenorrhea score was used to evaluate the efficacy. Results： The dysmenorrheal score in the embedment therapy group was much better than that in the control group after the 3 menstrual cycles（ P〈0. 01）. The short- term and long- term efficacies in the embedment therapy group were better than those in the control group（ P〈0. 05,P〈0. 01）. Conclusion： The staging acupoint embedment therapy achieves superior short- term and long- term efficacies as compared with the oral administration of fenbid in the treatment of primary dysmenorrheal,and the acupoint embedment therapy is economical and safe,which has a continuously curative effect.
