异型气管导管在内窥镜低温等离子下小儿鼾症手术中的应用研究  被引量:2

摘  要:目的评价异型气管导管插管全麻在低温等离子下小儿鼾症手术中的安全性及实用性。方法择期耳鼻喉科小儿鼾症手术284例,随机分为2组:异型气管导管组(A组,n=142),普通气管导管或加强型气管导管组(B组,n=142),观察插管前后及术中病人的MBP、HR、Sp O2、Pa CO2及通气情况。结果两组之间插管前后生命体征比较无明显差异(P>0.05);术中B组出现3例因导管滑入主支气管,引起Sp O2下降;5例因导管压瘪或扭曲,打折致Sp O2急剧下降,1例需紧急更换异型气管导管。2例因手术操作导致脱落。结论与普通气管导管或加强型气管导管相比,异型气管导管具有特定的弯曲度,容易固定且价格低廉,不影响通气量,有利于手术操作,在小儿鼾症手术中应用安全、可靠、实用,值得在临床推广。Objective: To estimate safety and practicability of tracheal intubation anesthesia with shaped endotracheal tube in operation of childhood patients with snoring under endoscope low-temperature plasma. Methods: 284 selective operations of childhood patients with snoring in ear-nose-throat department were divided into two groups: shaped endotracheal tube( A,n = 142),and common endotracheal tube or enhancement endotracheal tube( B,n = 142). MBP,HR,Sp O2,Pa CO2 and ventilation were observed Before and after tracheal intubation or intraoperation. Results: Vital signs did not show obvious difference between A and B( P〈0. 05),Sp O2 drops appeared in 3 patients of the B group because tube rolled in main bronchus,5 patients because tube telescoped or angulated,1 patient needed the change into shaped endotracheal tube,and the tube broke away in 2 patients because of surgical operation. Conclusion: Shaped endotracheal tube has specific curvature,and it is easy to fix and the price is moderate,doe not affect ventilation,which is safe and practicable in the operation on childhood patients with snoring,and it is worth of spreading.
