摘 要:目的探索凋亡相关基因Caspase-9(-1263A/G)多态性与中国汉族男性士兵的训练性下腰痛及疼痛程度的相关性。方法于某步兵团和某装甲团分别选取247名和245名中国男性汉族士兵,按诊断标准分为下腰痛组和对照组,对下腰痛组患者进行疼痛视觉模拟量表(visual analogue scale,VAS)评分。抽取全部研究对象的外周静脉血,提取白细胞DNA,聚合酶链式反应(polymerase chain reaction,PCR)扩增,DNA测序仪检测Caspase-9(-1263A/G)位点的基因型,分析该位点与训练性下腰痛是否存在相关性。结果在步兵群体中,下腰痛组AA基因型的分布频率明显低于对照组,而GG基因型的分布频率要高于对照组,组间差异均具有统计学意义(P<0.01);在装甲兵群体中,AA基因型在下腰痛组中的分布频率同样低于对照组,GG基因型高于对照组,组间差异均具有统计学意义(P<0.01)。Caspase-9(-1263A/G)位点与下腰痛士兵VAS评分的相关性分析表明,携带GG基因型的装甲兵患者的VAS评分更高,差异具有统计学意义(P<0.05),下腰痛更为严重,但在步兵群体中不存在这种相关性,差异无统计学意义(P>0.05)。结论 Caspase-9(-1263A/G)多态性与中国汉族步兵和装甲兵的训练性下腰痛相关,并且与装甲兵下腰痛患者的严重程度存在相关性。Objective To explore the association between Caspase-9(-1263A/G) polymorphism and military training related low back pain with different severities in Chinese Han male soldiers. Methods A total of 247 Chinese Han male soldiers were chosen from an infantry regiment and 245 were chosen from an armored regiment. All subjects were divided into case group and control group. Visual analogue scale( VAS) scores were recorded in case group. Genomic DNA was extracted from peripheral blood leukocytes of each subject using standard protocols. The DNA fragment containing the polymorphic sequence was amplified by polymerase chain reaction( PCR). PCR products were genotyped to determine the genotype of Caspase-9(-1263 A / G). Whether there was an association between the genetic locus and military training related low back pain was analyzed. Results In the infantry regiment,AA genotype was lower in case group than that in control group( P〈0. 01),and GG genotype in case group was higher than that in control group( P〈0. 01). In the armored regiment,AA genotype in case group was also lower than that in control group( P〈0. 01),and GG genotype in case group was higher than that in control group( P〈0. 01). The armor with GG genotype scored higher on VAS scores( P〈0. 05). However,there was no association between the polymorphism and pain severities among the infantry( P〉0. 05). Conclusion There is an association between Caspase-9(-1263 A / G) polymorphism and military training related low back pain in Chinese Han infantry and armor. In addition,Caspase-9(-1263 A / G) polymorphism was associated with pain severity in the armor.
