慢性心力衰竭患者血浆S100B蛋白水平的检测与临床意义

摘　　要：考察血浆脑钠素（BNP）、超敏C反应蛋白（hs-CRP）和S100B在不同程度充血性心力衰竭（cHF）的差异和与心力衰竭（心衰）的相关性。回顾性分析91例CHF患者的临床资料。按心功能Ⅱ级～Ⅳ级分成3个纽。检测并比较hs—CRP、BNP和S100B在不同程度心衰的差异，探讨其与心衰的相关性。结果随心衰程度增加，3个组CHF的S100B呈升高趋势（P-0．03）。3个组血浆BNP、hs—CRP、S100B以及左室射血分数（LVEF）的差异均有统计学意义（P〈0．05）。不同程度心衰的hs-CRP、BNP和S100B指标，与其LVEF呈显著负相关性（r=-0．15，-0．48，-0．82，P〈0．05）。hs-CRP、BNP和S100B共同参与了心衰的病程演变，3项指标可反映左室功能和心衰程度，临床可将其用于诊断心衰和评估病情。To explore the clinical significance of the indicators including plasma brain natriuretic peptide （BNP）, hypersensitive C-reactive protein （hs-CRP）, S100B and their correlation with different degree of congestive heart failure （CHF） patients. A retrospective analysis of data on 91 cases of patients with CHF, they were divided into 3 groups for research with heart function class of Ⅱ～Ⅳ. The levels of indicators including BNP, hs-CRP, S100B were evaluated by enzyme-linked immune method. Explore the differences of each index in different degrees of heart failure, and its correlation with different levels of heart failure. S100B levels in the 3 groups of patients with different levels of heart failure showed a trend of increase （P〈0.03）. Differences of plasma BNP, hs-CRP, S100B, and LVEF （%） in 3 groups were all statistically significant （P〈0.05）. Indexes of hs-CRP, BNP and S100B were has a significant negative correlation with LVEF （%） （r=-0.15,-0. 48,-0. 82,P〈0. 05）. All the Indexes of hs-CRP, BNP and S100B were in the course of evolution of the heart failure. The 3 indicators monitoring level can accurately reflect the situation and the left ventricular function, also it can he used in the diagnosis of heart failure and he good at evaluate condition of degree of heart failure.
