胎头娩出速度控制助产防护产妇会阴裂伤的价值

摘　　要：目的探讨控制胎头娩出速度助产法在减少产妇会阴裂伤中的价值。方法选择2014年1月至2014年12月行阴道分娩的120例产妇为研究对象，随机将其分为两组，每组60例，对照组产妇给予常规阴道助产保护，观察组产妇给予胎头娩出速度控制助产，比较两组会阴裂伤发生率及其裂伤程度、第二产程时间、产后出血量、住院时间。结果观察组会阴裂伤发生率、Ⅱ度裂伤率分别为18．33％、3．33％，与对照组（40．00％、24．33％）比较差异有统计学意义（P〈0．05）。两组产后出血量[（127．60±47．28）ml、（179．52±89．35）ml]及住院时间[（2．40±0．33）d、（3．43±0．62）d]比较差异有统计学意义（P〈0．05）。结论控制胎头娩出速度助产能明显减少产妇会阴裂伤发生率，减轻裂伤程度，缩短住院时间，值得临床推广。Objective To investigate the effect of the midwifery of controlling the speed of fetal head on reducing maternal perineal laceration. Methods From January 2014 to December 2014, 120 pregnant women with vaginal delivery were selected as the research objects, they were randomly divided into two groups through the method of digital form, each group contained 60 cases. The patients in control group were given the midwifery of routine vaginal maternal protection, and the patients in observa- tion group were given the midwifery of controlling the speed of fetal head, and then the incidence of perineal laceration, the de- gree of laceration, the second delivery process, the postpartum blood loss and the length of hospital stay of the two groups were compared. Results The incidence of perineal laceration and 11 degree laceration rate of the observation group respectively was 18.33% and 3.33% , and those of the control group was 40. 00% and 24. 33% , there were significant differences （P 〈0. 05）. In addition, the postpartum blood loss and the length of hospital stay of the observation group respectively was （ 127. 60 ± 47. 28 ）ml and （2.40±0. 33 ） d, the postpartum blood loss and the length of hospital stay of the control group respectively were （179.52±89.35） ml and （3.43±0.62） d, and the differences were significant （P〈0.05）. Conclusion The midwifery of controlling the speed of fetal head can reduce the incidence of maternal perineal laceration, alleviate the degree of laceration and shorten the hospital stay, it is worthy of promotion.
