摘  要:目的 研究外周血Th亚群细胞因子IFN-γ,IL-4和TGF-β1在急、慢性儿童特发性血小板减少性紫癜中的浓度及临床意义.方法 应用流式细胞术检测外周血IFN-γ和IL-4表达水平,采用酶联免疫(ELISE)法检测外周血单个核细胞生成的TGF-β1水平,并与24例正常对照组检测进行比较.结果 aITP患儿Th1细胞因子IFN-γ阳性百分比高于正常对照组(P<0.05),cITP患儿Th1细胞因子IFN-γ阳性百分比较对照组显著降低(P<0.01).aITP患儿Th2细胞因子IL-4与正常对照组比较差异无统计学意义(P>0.05),cITP患儿Th2细胞比对照组显著升高(P<0.05).aITP和cITPTh3细胞因子TGF-β1水平均较正常对照组明显降低,cITP较aITP显著降低,差异有统计学意义(P<0.05).结论 儿童急、慢性ITP中Th细胞因子对其发病机制研究及鉴别诊断有重要价值.Objective To study the determination and clinical significance of Th cytokines subsets of IFN-γ,IL-4 factor and TGF-β1 in acute and chronic idiopathic thrombocytopenic purpura. Methods The level of IFN-γ, IL-4 and TGF-β1 (pro- duced by peripheral blood mononuclear cells)were detected by flow cytometry and enzyme-linked immunosorbent assay (E- LISE),respectively,in patients and 24 controls. Results The positive percentage of factor IFN-γ in children with aITP was higher than that in the controls (P〈0.05). The positive percentage of factor IFN-γ in children with cITP was significantly lower than the controls (P〈0.01). The level of IL-4 had no statistical significance in children with aITP compared with controls. The level of IL-4 was higher significantly in children with cITP than that in controls (P〈0.05). The level of TGF- β1 in children with aITP and cITP was lower significantly than that in controls, that was more significance in cITP than that in aITP (P〈0.05). Conclusion The state of Th cytokines in children with acute and chronic ITP is of great value in the understanding of the pathogenesis and differential diagnosis.
