摘　　要：目的 通过检测人类白细胞抗原B27（HLA-B27）蛋白表达量,结合强直性脊柱炎（AS）的疾病活动指数（BASDAI）评分,探讨HLA-B27表达量可能在AS疾病诊疗中的意义.方法 收集120例临床疑似AS病例和50例健康体检者的一般临床资料,包括性别、年龄、家族史等,对所收集病例的全血采用磁珠酶联免疫法检测HLA-B27蛋白表达量.采用SPSS17.0数据分析软件进行分析,P＜0.05为差异有统计学意义.结果 120例疑似AS患者中,临床最终确诊为AS患者55例,其中人类HLA-B27检测阳性者53例（96.36％）;50例健康对照组中筛查HLA-B27检测阳性2例（4％）.53例HLA-B27阳性AS患者HLA-B27表达量与BASDAI进行相关性分析显示：BASDAI评分与HLA-B27蛋白质表达量（吸光度A值）呈正相关性（r=0.562,P=0.000）.结论 在AS患者中HLA-B27表达量与AS疾病活动指数之间存在相关性,可以为疾病的状态评估提供参考.Objective It was little known to the roles of the intensity of HLA-B27 in AS. To investigate whether the expres- sion intensity of HLA-B27 was associated with incidence of AS, the expression of HLA-B27 was detected in patients with AS. Methods 120 cases patients suspected with AS and 50 healthy volunteers were enrolled in the study. Main demographic and clinical characteristics of the subjects were collected, including age, sex, family history and diseaseaetivity index in AS. The expression of HLA-B27 protein were detected by immunomagnetic separation and enzyme-linked immunosorbent assay （IMS-ELSIA） in all subiects. All experimental data were analyzed by SPSS17.0 software and P values 〈0.05 were consid- ered to be significant. Results Firstly,there were 55 subjects that were finally diagnosed as AS patients among the 120 pa- tients suspected with AS. There were 53 subjects with H LA-B27 positive （96.36 % ） in 55 patients with AS. Among the 50 healthy volunteers,there were only 2 of them HLA-B27 positive （4%）. Further analysis showed that there was a positive between BASDAI and the expression intensity of HLA-B27 protein （r=0. 562,P=0. 000）. Conclusion There was a posi- tive correlation between the expression intensity of HLA-B27 and the BASDAI in AS subjects. The expression of HLA-B27 intensity could provide important information for estimating the condition of AS patients.
